- Abstract
The letter discusses the use of high-dose intravenous vitamin K in the management of hemostasis in cirrhosis. The author highlights that while the international normalized ratio (INR) is useful for stratifying disease severity, it does not predict hemorrhage risk in cirrhosis. The author suggests that clinicians should rely on more functional coagulation assessment methods, such as viscoelastic testing, to assess coagulopathy in this patient population. Additionally, the author questions the role of vitamin K in cirrhosis and cites limited evidence of its benefit, as well as potential harm associated with its use. The author concludes that the use of vitamin K in nonbleeding cirrhosis patients should be retired.
