The authors discuss the challenges in optimizing preexposure prophylaxis (PrEP) development, engagement, and access for human immunodeficiency virus prevention. Topics mentioned include the tension between innovation in biomedical science and its application in public health efforts, increase in sexually-transmitted infection incidence in a subset of PrEP users, and the link of PrEP use to higher educational attainment, income, and insurance status.
