Neurectomia do infraorbital como tratamento de headshaking em equino.

- Abstract
Headshaking é uma síndrome caracterizada pelo movimento repetitivo involuntário da cabeça, sendo vertical ou horizontal. A principal causa é pela hipersensibilidade do nervo trigêmeo, V par de nervo craniano, responsável por fornecer informações sensoriais da cabeça, olhos e pálpebras e desempenhar a função motora dos músculos faciais na mastigação. Como principal sintomatologia, observa-se irritação, coriza, epífora, fotosensibilidade e hipersensibilidade ao toque. Exames complementares como a endoscopia e a radiografia são fundamentais para se obter a causa primária da afecção, juntamente ao exame físico e histórico do paciente. O objetivo deste relato é descrever o tratamento cirúrgico para equinos acometidos por esta síndrome, através da neurectomia do infraorbital. Um equino da raça Quarto de Milha, macho, 10 anos de idade, foi encaminhado para procedimento cirúrgico de neurectomia do infraorbital por apresentar headshaking intermitente. O médico veterinário responsável pelo caso já havia avaliado o animal na propriedade. Após diagnóstico, inicialmente instituiu-se tratamento clínico, com aplicações de anestésico local perineural (cloridato de lidocaína 2% sem vaso constritor) associado a Shakeaze®. O equino apresentou melhora significativa, porém três meses após o tratamento voltou a apresentar a afecção de maneira intermitente. Após nova avaliação, optou-se por correção cirúrgica. O procedimento foi realizado sob anestesia geral inalatória. Utilizou-se alfa-2-agonista como sedativo, xilazina 10% (0,6 mg/kg/i.v.) como pré-anestésico e para indução anestésica administrou-se cetamina (2 mg/kg/i.v.) associada a midazolam (0,1 mg/kg/i.v.) e manutenção anestésica com isofluorano. Para a neurectomia, realizouse bloqueio perineural do infraorbital seguido de uma incisão de pele de mais ou menos 5 cm. Em seguida, foi feita divulsão de todo o tecido conectivo adjacente e exposição do músculo elevador do lábio. Para não lesionar a musculatura, rebateu-se utilizando um afastador de Farabeuf. Após identificação do nervo no forâme infraorbital, realizou-se divulsão e exposição do mesmo, seguida por uma neurectomia em guilhotina, removendo mais ou menos 4 cm da estrutura. Aproximou-se tecido subcutâneo com fio absorvível 2-0 em padrão simples separado e sutura da pele com fio não absorvível 1 em padrão simples separado. Após o procedimento, o tratamento instituído foi flunixin meglumina (1,1 mg/kg/i.v./sid/5 dias), gentamicina (6,6 mg/kg/i.v./sid/3 dias) associado à limpeza da ferida cirúrgica com solução de hipoclorito de sódio e aplicação tópica de arnica. Após 7 dias, o animal obteve alta hospitalar e não houve relatos da volta do headshaking durante atividade atlética.
