Teranóstico: novas possibilidades em nanomedicina.

- Abstract
Introdução: O termo teranóstico é a combinação de dois termos amplamente usados, terapia e diagnóstico, e vem consolidando-se como novo campo da Medicina. Esse neologismo aplica-se à nanomedicina, que usa nanomateriais agindo ao nível supramolecular, receptores, canais iônicos, proteínas e ácidos nucléicos. Metodologia: Pesquisa nas bases de dados MEDLINE e LILACs, usando o termo theranostic. A busca foi limitada a estudos de coorte e de casos e controle e como resultado foram encontrados 6 estudos, mas apenas 4 se encaixaram na proposta da presente revisão. Discussão: As nanopartículas possuem características específicas, como área superficial maior, que permite a incorporação de moléculas em sua superfície, adicionando novas propriedades a elas. Radiomarcadores podem ser incorporados aos nanomateriais para formação de imagens, por exemplo, com finalidade diagnóstica. Ao mesmo tempo princípios ativos podem ser nanoencapsulados para impedir sua desestabilização no plasma ou sua absorção em outros tecidos que não sejam o tecido alvo. Por isso os nanomateriais podem ser usados simultaneamente para diagnóstico, tratamento e monitoramento da resposta terapêutica. Tendo em vista esse potencial, estudos têm sido realizados a fim de encontrar potenciais biomarcadores para serem usados com essa finalidade. Estudos em animais mostraram que a albumina-AGE pode ser usado como biomarcador para terapia e diagnóstico de doenças neurodegenerativas como a doença de Parkinson. A proteína S100A9 foi identificada como uma interessante possibilidade de biomarcador para resposta terapêutica na artrite reumatóide. DKK-1 foi identificado como um biomarcador útil para o rastreio de alguns tipos de câncer, como pulmão e esofágico e os anticorpos anti-DKK1 foram identificados como potenciais ferramentas para o diagnóstico e terapia desses cânceres. Genes do receptor nicotínico de acetil-colina foram identificados como potenciais alvos teranósticos para o câncer de pulmão. Conclusão: A nanomedicina é uma área relativamente recente, mas que já traz consigo uma nova ferramenta para medicina, a junção do diagnóstico e da terapia em um mesmo ativo. Novos biomarcadores que sirvam a esse propósito têm sido pesquisados, inclusive para doenças em que ainda não se tem um tratamento tão eficaz. Portanto, a pesquisa nessa área precisa ser incentivada e desenvolvida para a obtenção de um diagnóstico mais precoce e um tratamento mais efetivo em diversas afecções.
