- Abstract
The article presents information on the medical case of a 39-year-old man with no cardiac structural abnormalities and a 3-year history of paroxysmal atrial fibrillation (AF). At electrophysiologic study, typical atrial flutter ablation is performed and the AV node slow pathway ablated. During that study, AF initiates spontaneously from a rapidly firing focus with earliest activation in the left atrial leads. The patient is referred for focal AF ablation. The patient remains arrhythmia-free off all medications 4 months postprocedure, despite daily episodes before ablation. This case demonstrates the potential feasibility of this technique in a patient with a focus in the PV near the ostium.
