- Abstract
Objective To analyze the effect of pharmacists′ intervention on the banned drugs in patients with severe renal impairment before and after the establishment of the clinical pathway jointly by pharmacists and physicians, and to explore the effective ways for clinical pharmacists to promote clinical rational drug use . Methods From January to June 2021,clinical pharmacists reviewed and intervened in the prohibited drugs in the patients with severe renal dysfunction in the Nephrology Department of Xuanwu Hospital of Capital Medical University, and recorded the intervention results .In July 2021,the clinical use pathway of the intervened drugs was established through medical communication .From August 2021 to January 2022,the intervened drugs were reviewed and intervened according to the clinical pathway; the intervention results were recorded and compared with those from January to June 2021 .Results From January to June 2021,pharmacists found that 9 drugs banned in the instructions were used by patients with severe renal impairment and gave interventions. Five drugs were refused to be discontinued by physicians, and 4 were discontinued, but all were re-used after interventions . From August 2021 to January 2022,the 9 drugs that were previously intervened were reviewed according to the clinical pathway, and only two drugs were used, which were intervened, and both interventions were accepted and none of them were re-used .Conclusion The establishment of clinical pathways by pharmacists and physicians may be an effective way to promote clinical rational drug use.
