- Abstract
El hipotiroidismo subclínico (HSC) es un trastorno endocrino común caracterizado por niveles elevados de hormona estimulante de la tiroides (TSH) con niveles normales de T4 libre. El diagnóstico de estos pacientes se basa, fundamentalmente, en pruebas de funcionamiento tiroideo, las cuales pueden ser acompañadas con test de detección de anticuerpos anti-peroxidasa tiroidea. Los niveles de TSH son esenciales para el correcto abordaje terapéutico de los pacientes con HSC. El tratamiento tiene como fin la normalización de las concentraciones séricas de TSH. No obstante, la terapia de reemplazo hormonal con levotiroxina aún se encuentra en constante polémica, por lo que es necesario el desarrollo de más ensayos clínicos aleatorizados controlados por placebo que permitan no sólo dilucidar las dudas sobre los efectos de este fármaco en el HSC y sus comorbilidades, sino también determinar el correcto abordaje terapéutico de esta enfermedad. Esto es particularmente importante para poblaciones especiales, como los adultos mayores, y las mujeres embarazadas y en el puerperio, entre otros. Es por ello que en el presente artículo se abordarán los pasos a seguir para establecer el diagnóstico adecuado del HSC, y se describirá la evidencia clínica reportada sobre el uso de la terapia de reemplazo hormonal con levotiroxina en pacientes con HSC. Subclinical hypothyroidism (SCH) is a frequent endocrine disorder characterized by high levels of thyroid-stimulating hormone (TSH) with normal levels of free T4. The diagnosis of these patients is base, fundamentally in thyroid function tests, which may be accompanied with the measurement of anti-thyroid peroxidase antibodies. Determination of TSH levels is essential for the correct therapeutic approach to patients with SCH. The treatment aims to normalize serum TSH concentrations. However, hormone replacement therapy with levothyroxine remains in constant controversy. Therefore, it is necessary to develop novel randomized, placebo-controlled clinical assays that may allow the elucidation of doubts about the effects of these drugs in SCH and its comorbidities, as well as the determination of the appropriate therapeutic management. This is particularly important in special populations, such as the elderly, and women during pregnancy and the postpartum, among others. Thus, in this article, we will revise the relevant aspects for the diagnosis of SCH, and we will describe the clinical evidence available on the use of hormone replacement therapy with levothyroxine in patients with SCH.
