Abordagem clínico cirúrgica de intussuscepção ceco cólica em Mini Horse.

- Abstract
A intussuscepção ceco cólica consiste na invaginação cecal através do orifício ceco-cólico em direção ao cólon ventral direito. Recomenda-se a correção cirúrgica e o prognóstico torna-se desfavorável em casos que há necessidade de tiflectomia. Objetivou-se relatar um caso de intussuscepção ceco cólica em potro Mini Horse, macho, 7 meses de idade e 36 kg, que encontrava-se com sinais de desconforto abdominal, com início do quadro nas 12 horas anteriores. O animal possuía dor intensa, taquicardia (72 bpm), hipomotilidade intestinal e mucosas congestas, lactato sérico 4,5 mMol e peritoneal 5,4 mMol, hematócrito 33% e proteína plasmática total de 7,8 g/dL; sondagem nasogástrica improdutiva, apresentando intestino delgado distendido na avaliação ultrassonográfica, cólon repleto de gás e ceco com parede espessada. Considerando a evolução do quadro clínico e demais achados nos exames complementares, o paciente foi encaminhado à celiotomia exploratória. Como medicação antibiótica e anti-inflamatória précirúrgica, administrou-se penicilina procaína (22.000 UI/kg/IV), gentamicina (6,6 mg/kg/IV), flunixim meglumine (1,1 mg/kg/IV), seguida de sedação com xilazina 10% (0,7 mg/kg/IV). A indução anestésica deu-se por cetamina 10% (2,2 mg/kg) e diazepam (0,1 mg/kg), com manutenção sob anestesia inalatória com isoflurano e oxigênio a 100% conforme demanda. Durante o transoperatório foi realizada a administração de infusão intravenosa de lidocaína (bolus: 1,3 mg/kg; infusão contínua: 0,05mg/kg) e dobutamina (0,002 mg/kg). Logo após a incisão na linha alba e exteriorização do colon foi possível observar a intussuscepção ceco cólica. O cólon ventral foi incidido e o intuscepto exposto. Visto o comprometimento vascular cecal, optou-se por realizar a tiflectomia parcial de 2/3 do corpo. Realizou-se uma sutura simples contínua, amputação da porção comprometida e eversão do coto. Após a eversão, realizou-se uma segunda sutura invaginante. Em seguida, realizou-se enterorrafia do cólon e celiorrafia. Na terapia pós-operatória, administrouse penicilina procaína e gentamicina por 6 dias, 4 dias de flunixim meglumine e metronidazol (15 mg/kg/IV), omeprazol e probiótico durante 11 dias. Além disso, realizou-se infusão intraperitoneal com 1g de ceftriaxona e heparina por 2 dias e infusão acrescida de DMSO 10%, com o objetivo de evitar aderências e peritonite. A intussuscepção ceco cólica é pouco relatada em animais adultos e mais frequentes em potros. Relaciona-se com hiperperistaltismo, o que pode ter sido gerado devido à troca de manejo que o potro sofreu no período anterior ao episódio de cólica, constatado na anamnese. Dentre as complicações pós-operatórias, estão o íleo adinâmico, peritonite, infecções incisionais e cólica. Conclui-se que, apesar do procedimento cirúrgico ser desafiador e o prognóstico desfavorável, a abordagem emergencial e a adequada realização do procedimento foram eficazes para o tratamento de intussuscepção ceco cólica e sobrevida do animal.
