Challenges in the Administration of Solid Oral Dosage Forms Among Geriatric Patients in Long Term Care Facilities in Jamaica.

- Abstract
Geriatric residents of long term care facilities (LTCFs) are most often suffering from one or more non-communicable diseases (NCDs). NCDs require longterm treatment with varied medicines that are predominantly formulated as solid oral dosage forms (SODFs). These SODFs are commercially available as either modified release drug delivery systems (MRDDS) or immediate-release drug delivery systems IRDDS. This study examined and documented the practice among caregivers in administering MRDDS to geriatric patients who have difficulty swallowing. A descriptive research design was used. The purposive sampling technique was used to select 18 nursing homes in Kingston and St. Andrew. Data were collected through a questionnaire and analyzed using Microsoft Excel and SPSS programs. The response rate was 15 (83.3%). All of the responders reported alteration of MRDDs without regard for dosage form design. The justifications presented for altering SODFs were the refusal of patients to take the drug and dysphagia. LTCFs geriatric residents are receiving altered MRDDS without routine monitoring for medication misadventures. Training of caregivers needs to be given on drug administration. This is an opportunity for pharmacists to provide training for LTCFs staff and increase pharmaceutical care for geriatric patients.
