Efeitos Cardiovasculares do Uso de Cocaína.

- Abstract
Objetivo: O presente trabalho tem como objetivo alertar sobre as complicações cardiovasculares decorrentes do uso de cocaína, droga que vem apresentando um consumo crescente nas últimas décadas, principalmente por parte dos jovens, além de apresentar formas de tratamento para os efeitos advindos de seu consumo. Metodologia e fonte de dados: Foi realizada uma revisão de artigos publicados entre 2007 e 2017 a respeito do tema, sendo consultadas as seguintes bases de dados: SciELO, PubMed, MEDLINE e C APES. Foram utilizados como descritores os termos: cocaine, cardiovascular effects, sympathomimetic agent, cardiotoxicity, myocardial infarction e cardiomyopathy. Discussão: Nos últimos 30 anos, tem-se verificado um aumento alarmante no consumo de cocaína em todo o mundo. O uso dessa droga está relacionado a uma série de complicações cardiovasculares, entre as quais se destacam a angina pectoris, o infarto do miocárdio, cardiomiopatias, endocardite, arritmias, aterosclerose e dissecção de aorta. Tais eventos são devidos a um conjunto de alterações fisiológicas induzidas pela cocaína no sistema cardiovascular. Sua ação simpaticomimética decorre do bloqueio da recaptação de catecolaminas e do aumento da sensibilidade adrenérgica a norepinefrina, o que resulta na elevação da frequência cardíaca, da pressão arterial, da contratilidade e, por conseguinte, da demanda por oxigênio. Em contrapartida, ocorre uma diminuição no suprimento de oxigênio em função da vasoconstrição induzid a pela droga, fenômeno que, uma vez ocorrido nas coronárias, pode levar ao infarto do miocárdio. Outro efeito que merece destaque são as cardiomiopatias associadas a problemas na geração e condução de potenciais de ação, decorrentes do bloqueio de canais iônicos. Diante dessas alterações, alguns medicamentos são indicados para seu tratamento: benzodiazepínicos, eficazes no alívio da angina; lidocaína, um bom agente anti-arrítmico; e vasodilatadores diretos, como a fentolamina. Os beta-bloqueadores são contra-indicados por potencializarem a estimulação adrenérgica da cocaína. Conclusão: Em virtude da presença crescente, nos pronto-socorros, de pacientes apresentando complicações cardiovasculares decorrentes do uso de cocaína, o conhecimento dos mecanismos de ação da droga e da forma como ela expressa sua toxicidade é de suma importância para a realização do diagnóstico correto e para o estabelecimento da melhor linha de tratamento, campos estes que merecem ser objetos de maior es pesquisas para o seu constante aperfeiçoamento.
