Dear Editor, The prognosis of extramammary Paget disease (EMPD) is favourable if tumour cells are localized to the epidermis.[1] However, patients with dermal and lymphatic invasion are at risk of metastasis, which is associated with poorer prognosis as no effective systemic therapy has been established.[[2]] Patients with advanced EMPD present with characteristic metastatic patterns, such as underpants-distribution erythema and severe lymphoedema of the lower extremities resulting from lymphatic obstruction.[5] Therefore, as the prognosis and clinical characteristics of advanced EMPD are strongly associated with lymphatic spread, lymphatic pathways may represent the main metastatic pattern of EMPD. (c) The nests of Paget cells have plugged the hepatic sinusoids, vessels or lymph vessels and displaced normal hepatic cells (haematoxylin and eosin stain; original magnification × 200). gl.
