- Abstract
Introducere. În ciuda faptului că medicina se af lă într-un progres continuu, iar metodele de diagnostic au devenit, pe parcursul ultimilor ani, mai accesibile și mai eficiente din punct de vedere tehnologic, endometrioza rămâne o patologie subdiagnosticată. De etiologie neelucidată, aceasta înglobează o multitudine de forme clinice, ce pot îndruma clinicianul către un diagnostic eronat. Se consideră că debutul bolii are loc în timpul adolescenței, astfel încât simptome precum dismenoreea, dispareunia sau durerea pelviană pot orienta diagnosticul către o tulburare de ciclu menstrual, respectiv boală inflamatorie pelviană, iar tulburările gastrointestinale, către o patologie în afara sferei ginecologice. Astfel, diagnosticul precoce, bazat pe experiența clinicianului, pe anamneză și investigații paraclinice, până la examenul histopatologic, este obligatoriu pentru prevenția progresiei bolii către forme extensive, invalidante. Materiale și metodă. Lucrarea reprezintă o metaanaliză a articolelor publicate în reviste de specialitate și pe baza experienței personale. Rezultate. Atât tratamentul cu agoniști GnRH, COC, SEPM şi progestative, cât și cel chirurgical reduc durerea pelviană. Cu toate acestea, în aproximativ 70-80% din cazurile de durere pelviană neresponsivă la tratamentul medicamentos, s-a stabilit anatomopatologic diagnosticul de endometrioză. Concluzii. Conduita medicală în endometrioză rămâne o provocare. Deși tratamentul chirurgical poate fi atât diagnostic, cât și terapeutic, acesta nu tratează cauza bolii, ci doar leziunile produse, adică efectul. În cazul adolescentelor, mai ales în formele extensive, cu afectare intestinală, complicațiile intra- și postoperatorii pot afecta semnificativ calitatea vieții.
