- Abstract
Wstęp Stale narastająca lekooporność bakterii wywołujących zakażenia jest jednym z głównych czynników wydłużających hospitalizację, zwiększających jej koszt oraz wpływających na występowanie powikłań u pacjentów. Osoby bezdomne ze względu na częste urazy, a także brak środków na leczenie lub jego zaniechanie stanowią jedną z grup, w której występuje oporność bakterii na antybiotyki. Rany bezdomnych są często zaniedbane, zanieczyszczone, a opatrunki są rzadko zmieniane lub w ogóle niewykonywane. Ponadto u osób bezdomnych występują często schorzenia współistniejące, których leczenie także bywa zaniedbywane, co wydłuża proces gojenia ran i zwiększa prawdopodobieństwo wystąpienia powikłań infekcyjnych, często prowadzących nawet do amputacji kończyny. Bezdomni są hospitalizowani w placówkach medycznych z innymi pacjentami, co sprawia, że problem zakażeń nie zamyka się tylko w obrębie tej grupy, ale stanowi istotną kwestię dla całego systemu opieki medycznej. Cel Celem badania była identyfikacja szczepów występujących w ranach kończyn dolnych osób bezdomnych i ocena wrażliwości tych szczepów na antybiotyki oraz wybrane środki dezynfekcyjne. Celem nadrzędnym było stworzenie podstaw do dyskusji na temat empirycznej terapii zakażeń u osób bezdomnych, tak by nie zwiększać problemu lekooporności oraz zapobiegać potencjalnym ogniskom epidemicznym. Materiał i metody Wymazy pobrano z ran kończyn dolnych od 44 osób bezdomnych przebywających na terenie Krakowa. Szczepy bakterii zidentyfikowano przy zastosowaniu komercyjnych testów biochemicznych oraz metody MALDI-TOF. Ocenę lekowrażliwości wykonano zgodnie z wytycznymi EUCAST (ang. European Committee on Antimicrobial Susceptibility Testing). Zbadano również wrażliwość na najczęściej stosowane do odkażania ran głębokich środki dezynfekcyjne (Octenispet®, Microdacyn®) lekoopornych szczepów Staphylococcus aureus wyizolowanych z ran bezdomnych oraz szczepu wzorcowego S. aureus ATCC6538. Badanie wykonano według normy EN 13727:2012+A1:2013. Wyniki Z ran wyizolowano 102 szczepy, spośród których najliczniejszą grupę stanowiły gronkowce (42,2%; n=43), w tym S. aureus (n=32) i gronkowce koagulazo-ujemne (n=11), bakterie z rodziny Enterobacteriaceae (25,5%; n=26), paciorkowce (25,5%; n=26) oraz rodzaj Pseudomonas (6,9%; n=7). Wśród gatunku S. aureus najwięcej szczepów wykazało oporność na makrolidy i linkozamidy, podobnie wśród gronkowców koagulazo-ujemnych (53,1%; n=17 vs. 72,7%; n=8). Wśród Pseudomonas sp. wykazano obecność fenotypu MBL u jednego szczepu (14,3%). W badaniu środków dezynfekcyjnych Microdacyn® wykazał lepszą skuteczność niż Octenisept® (odpowiednio 72,7% vs. 54,5%). Wnioski Oporność na antybiotyki bakterii izolowanych z ran osób bezdomnych stanowi problem terapeutyczny oraz epidemiologiczny. W badaniu bakterie oporne przynajmniej na jedną grupę antybiotyków izolowano z rany u co trzeciego pacjenta. Obecnie stosowana antybiotykoterapia empiryczna - w świetle uzyskanych wyników - może być nieskuteczna, szczególnie przy tak dużym odsetku szczepów opornych na antybiotyki pierwszego rzutu. Mimo to trzeba mieć świadomość, że obecnie opieka medyczna nad osobami bezdomnymi jest dosyć trudna i z tego względu istnieje konieczność wypracowania pewnych schematów leczenia w tej grupie pacjentów. Badania środków dezynfekcyjnych podjęto ze względu na fakt, iż często w grupie osób bezdomnych leczenie zakażeń ran ogranicza się do oczyszczenia i odkażenia rany. Z uzyskanych danych wynika, że Microdacyn® jest skuteczniejszym środkiem i należy preferować ten preparat w dezynfekcji ran głębokich zamiast Octeniseptu®. Badania środków dezynfekcyjnych były prowadzone w czasie, gdy następowały zmiany charakterystyki produktu leczniczego Octenisept®.
