Les nouvelles thérapies nutritionnelles dans la maladie de Crohn. Que retenir de l'expérience pédiatrique ?

Résumé: Il est désormais reconnu que l'environnement joue un rôle prépondérant dans le développement de l'inflammation observée dans les maladies inflammatoires chroniques de l'intestin (MICI). Parmi les facteurs environnementaux, le rôle de l'alimentation est établi. Les comités d'experts pédiatriques recommandent chez l'enfant la nutrition entérale exclusive (NEE) en traitement d'induction de la maladie de Crohn (MC) légère à sévère sans lésions anopérinéales. Récemment, des régimes d'exclusion moins contraignants ont été proposés, afin d'améliorer l'acceptabilité de la NEE. Si certains régimes demandent encore une évaluation à plus grande échelle, d'autres semblent déjà très prometteurs. La prise en charge nutritionnelle de la maladie de Crohn, validée en pédiatrie, doit pouvoir être proposée aux patients adultes. Ces nouvelles approches thérapeutiques nécessiteront ainsi de nouveaux moyens humains et financiers déployés dans la prise en charge diététique et dans l'éducation thérapeutique des patients atteints d'une MC. Environmental factors play a critical role as trigger of inflammation in inflammatory bowel disease (IBD) and nutritional factors contribute to pathology of IBD. Moreover, exclusive enteral nutrition (EEN) is now considered, as the first line induction therapy for mild to severe paediatric Crohn disease (CD) without anoperineal disease. Recently, new therapeutic nutrition protocols such as "Crohn disease exclusion diet" have been prospectively evaluated in paediatric CD patients. Some of them reported promissing results. Yet, these new therapeutic options, validated in paediatric, could also be proposed to adult patients. Overall, new nutritional approaches will require new financial resources for dietetician and therapeutic education founding.
