- Abstract
Das Lungenkarzinom ist eine histologisch, immunologisch und daher auch morphologisch-funktionell sehr unterschiedliche Gruppe von Neoplasien mit weltweit höchster Krebsmortalität. Daher ist auch die Palette der lungenkarzinomimitierenden Erkrankungen sehr breit und umfasst angeborene, infektiöse und inflammatorische Veränderungen, benigne Raumforderungen bis hin zu anderen primären und sekundären Neoplasien. Die Schwierigkeit für die Radiologie liegt im Rahmen der Primärdiagnose darin, das Lungenkarzinom mit hoher Treffsicherheit zu diagnostizieren, unter Berücksichtigung der Grenzen der spezifischen Diagnose, aber auch in Kenntnis „klassischer" Fallstricke und seltener Entitäten, die ein Lungenkarzinom imitieren können. Die Einengung der Differenzialdiagnose bedarf dabei einer engen interdisziplinären Kooperation unter Einbeziehung von Klinik und Anamnese des Patienten. Eine exakte Analyse des Computertomographie(CT)-Musters und der Verteilung der Raumforderungen sowie das Beachten von zusätzlichen Veränderungen und Befall anderer Organsysteme können der Schlüssel zur Diagnose sein. Die Fluordesoxyglukose(FDG)-Positronenemissionstomographie(PET)-CT ist bei wenigen Imitatoren des Lungenkarzinoms hilfreich.
