- Abstract
Chronic neck pain originating from the cervical facet joints is common. The history and the clinical examination are important in order to assign the complaints to a movement segment, but the diagnosis is established by interventional procedures. Imaging is also not suitable for identifying a painful facet joint. Fluoroscopic-guided medial branch blocks are therefore used for diagnosis. The two nerves supplying the specific joint are temporarily blocked with a local anesthetic. It is discussed which local anesthetics should be used and whether a medial branch block needs to be repeated. If facet joint pain is detected, radiofrequency neurotomy of the medial branch can be considered as a specific therapy. Success can be expected in about two thirds of the patients for many months.
