- Abstract
Giriş: Bruselloz, tüm sistemleri etkileyip farklı klinik tablolara yol açan, tanı ve tedavisinde güçlükler yaşanabilen zoonotik bir hastalıktır. Sıklıkla kas iskelet sistemi tutulsa da genitoüriner komplikasyonlar yaklaşık %2-10 oranında görülür; en sık izlenen genitoüriner komplikasyon epididimoorşittir. Bu yazıda akut epididimoorşitlerin ayırıcı tanısında brusellozun da düşünülmesi gerektiğini vurgulamak amacıyla Burusella epididimoorşit olgusu sunulmuştur. Olgu: Yirmi dokuz yaşında erkek hasta dış merkez üroloji polikliniğine ateş sol testiste şişlik kızarıklık ağrı şikayeti ile başvurmuş. Akut epididimoorşit tanısıyla intramusküler seftriakson reçete edilmiş. Üç gün sonra şikayetleri gerilemeyen hasta enfeksiyon polikliniğine başvurdu. Fizik muayenede ateş: 38,4 ºC sol testis ağrılı, şiş ve kızarıktı, diğer sistem muayeneleri normaldi. Laboratuvar bulgularında beyaz küre: 6.600/mm3, CRP: 25 mg/lt, idrarda lokosit negatif, Brusella Coombs aglütinasyon testi 1/1.280 pozitif. Skrotal Doopler USG: Sağ epididim normal boyutlarda ve eko yapısındadır. Sol skrotum deri, deri altı doku kalın izlenmiştir. Sol spermatik kord sağa göre geniş izlendi. Sol epididim başı düzeyinde 23x13 mm çapında anekoik kist izlendi. Sol epididim ve testiste RDUS ile incelemede kanlanma artışı mevcuttur (akut epididomoorşit?) şeklinde yorumlanmış. Hastaya streptomisin 1 gr/gün, rifampisin 600 mg/gün, doksisiklin 200 mg/gün tedavisi başlandı. Tedavinin üçüncü gününde ateşi düşen onuncu gününde kliniği düzelen hastanın streptomisin tedavisi 2 haftaya rifampisin ve doksisiklin tedavisi 6 haftaya tamamlandı. Sonuç: Bruselloz endemik olan ülkemizde özellikle ampirik tedaviye yanıt vermeyen epididimoorşitlerin ayırıcı tanısında düşünülmesi gereken bir hastalıktır. Bu durum hastanın gereksiz ve yanlış tedaviden korunması için önemlidir.
