Las variantes genéticas en el gen AIP modulan la edad de inicio por sexo en casos de adenoma hipofisiario.

- Abstract
Objetivo: determinar si las mutaciones en el gen AIP se asocian a edad de inicio temprano. Antecedentes: las mutaciones de la línea germinal del gen AIP causan adenoma hipofisario, contribuyendo con el 15% de los tumores intracraneales aproximadamente. Estas mutaciones y la pérdida del alelo normal en tumores sugieren que AIP actúa como un gen de supresión tumoral. Dichas variantes genéticas incluyen deleciones, inserciones, sin sentido, y mutaciones en el sitio de empalme que resultan en una proteína trunca. Por lo general, los pacientes con adenoma hipofisiario tienen edad de inicio < 30 años. Métodos: se incluyeron 61 muestras de DNA genómico extraído de sangre periférica de pacientes mexicanos jóvenes (inicio < 40 años) con adenomas hipofisiarios. La región codificante, los límites exón-intrón, y las regiones no traducidas (UTR) fueron amplificadas por PCR y analizadas mediante secuenciación Sanger en busca de variantes genéticas en el gen AIP.Núm. de registro del protocolo: 35/15. Resultados: se identificaron 54 mutaciones que incluyen cambios sin sentido, sinónimos, deleciones, sitios de splicing, variables de significado incierto (VUS), así como regiones UTR-5' y UTR-3'. El 57.4% (35 sujetos) fueron portadores de variantes patogénicas (patógenas, probablemente patógenas y posiblemente patógenas); 18 pacientes (29.5%) presentaron variantes en regiones de interés (UTR-5', splicing y UTR-3'); 9 pacientes (14.8%) tuvieron variantes sinónimas, y 7 sujetos (11.5%) presentaron variantes benignas. Los portadores de variantes patógenas (p.His120Pro, p.Thr231Met, p.Arg81Gln) iniciaron la enfermedad a edad más temprana que los no portadores (P< 0.05) (ver Tabla). Existen diferencias significativas en edad de inicio entre hombres y mujeres portadores de mutación. Conclusiones: las mutaciones en AIP modulan la edad de inicio entre portadores y no portadores por sexo, según la región proteica que se vea afectada, y conducen a cambios adversos en funciones celulares como plegamiento proteico, estabilización proteica y metabolismo de xenobióticos.
