Hepatitis A Presenting as Pleural Effusion.

- Abstract
Introduction: Hepatitis A virus (HAV) infection is highly endemic in Indian subcontinent with high prevalence rates in school going children. Extra hepatic complications are rarely encountered and need high index of suspicion. Case Presentation: A female child of four years of age admitted with fever, icterus and right hypochondriac pain was found to have right sided pleural effusion. The child was managed symptomatically. She became afebrile by day two of admission and was discharged to home. Discussion: HAV is predominantly an asymptomatic infection associated with young children in non immunised population. Extra hepatic manifestations such as pleural effusion, transient ascites, cutaneous vasculitis, cryoglobulinemia, hemophagocytic syndrome, aplastic anemia, thrombocytopenia, transverse myelitis, acute tubular necrosis, nephrotic syndrome and acute glomerulonephritis, have been reported in few cases. Pleural effusion has been attributed to deposition of immune complexes. Conclusion: In any child presenting with pleural effusion associated with jaundice, Hepatitis A infection should be strongly considered in differential diagnosis.
