- Abstract
We read with great interest the article published by Artemiou et al.,1 which demonstrated that advantages of preoperative venoarterial extracorporeal membrane oxygenation (VA-ECMO) in patients with postmyocardial infarction ventricular septal defect (post-MI VSD). In the case of post-MI VSD, inadequate LV decompression can also worsen the right ventricle (RV) strain.3 Alternatively, Impella 5.5 (6.2 L/min) provides complete unloading of the LV. Our early experience in two patients with acute repair of anterior and posterior post-MI VSD and transaortic implantation of Impella 5.5 resulted in excellent recovery.
