EFFICACY OF INTERMITTENT PNEUMATIC COMPRESSION IN MANAGEMENT OF FILARIAL LYMPHEDEMA.

- Abstract
The management of filarial lymph edema is largely conservative. Mobilization of stagnant lymph from the interstitial spaces and prevention of secondary infection have a key role in overall management. We hereby present our evaluation of the efficacy of intermittent pneumatic compression in the patients with filarial lymph edema in terms of frequency and severity of the attacks of Dermatolymphangioadenitis (DLA), changes in cutaneous micro flora and alteration in the structure of skin and subcutaneous tissue besides the change in the limb girth. All the patients were subjected to sequential pneumatic compression with pressure ranging from 70 to 125 mm Hg for one hour daily for six days in a week fortnightly for 6 months. The limb girth was measured at the predefined 5 points at different levels in the extremity before and after the compression. Detailed history was taken at the end of each month for the number of attacks of dermatolymphangioadenitis (DLA). Punch cutaneous biopsy and the surface culture were taken before and at the end of 6 months of compression. Histological features and immunohistochemistry were also evaluated. The limb circumference in edematous limbs was significantly higher than in the age and sex matched control (Table 1). There was significant reduction in the limb girth at all the levels after 6 months of Intermittent pneumatic compression (Table 2). However, it still remained significantly higher than in control (Table 3). There was significant improvement in number and frequency of attacks of DLA, surface micro flora, and infiltration of CD4+ and CD 34+ cells. In conclusion, Intermittent pneumatic compression is very effective in reducing the limb girth, reducing the attacks of DLA and improving the texture and pliability of skin as proved by histologic features.
