- Abstract
Los autores investigaron el trastorno por estrés agudo y el posterior desarrollo de un trastorno por estrés postraumático (TEP) en niños y adolescentes que sufrieron una agresión sexual o un accidente de tráfico. Método: Los autores entrevistaron a 93 pacientes de 10-16 años que habían sido asistidos en un servicio de urgencias por haber sufrido una agresión sexual o un accidente de tráfico, antes de transcurridas 4 semanas de la agresión o del accidente, para evaluar la existencia de un posible trastorno por estrés agudo. A los 6 meses, se reentrevistaron 64 pacientes (68,8%) para evaluar la presencia de TEP. Resultados: En la entrevista inicial, 18 (19,4%) de los 93 pacientes padecían un trastorno por estrés agudo, y 23 (24,7%) cumplían todos los criterios diagnósticos de trastorno por estrés agudo excepto el de disociación. A los 6 meses, 8 de los 64 pacientes (12,5%) presentaban un TEP. La prevalencia de trastorno por estrés agudo y TEP no difirió entre los pacientes que habían sido agredidos sexualmente y los que habían padecido un accidente de tráfico. Las estadísticas de sensibilidad y especificidad y el modelado por regresión revelaron que el diagnóstico de trastorno por estrés agudo constituía un factor predictivo eficaz de un futuro TEP y que la disociación no tenía un papel significativo. Conclusiones: Se ha demostrado que el trastorno por estrés agudo constituye un factor predictivo eficaz de un futuro TEP en niños y adolescentes, mientras que la utilidad del criterio diagnóstico de disociación parece dudosa.
