- Abstract
Rehabilitacja kardiologiczna jest integralną częścią opieki kardiologicznej po przebytym ostrym zespole wieńcowym, która nie tylko zmniejsza ryzyko wystąpienia powikłań, ale również poprawia jakość życia pacjenta. W 2017 roku uruchomiono w Polsce program kompleksowej opieki specjalistycznej nad pacjentem po zawale mięśnia sercowego (KOS‑Zawał), który zapewnia pacjentowi 12‑miesięczną opiekę ambulatoryjną z dostępem do rehabilitacji pozawałowej. CEL Ocena pacjentów włączonych do programu KOS‑Zawał w aspekcie uczestnictwa lub absencji na rehabilitacji pozawałowej i możliwych czynników mogących wpływać na decyzje o skierowaniu lub niezalecaniu rehabilitacji przez kardiologa. METODY Do badania retrospektywnie włączono 297 kolejnych pacjentów zgłoszonych do programu KOS‑Zawał, którzy podzieleni zostali na dwie podgrupy w zależności od jednego kryterium – skierowania lub nieskierowania pacjenta na rehabilitację kardiologiczną. W obu podgrupach porównano obraz kliniczny, wywiad oraz wybrane parametry laboratoryjne i echokardiograficzne. WYNIKI 144 pacjentów (48,5%) skierowano na rehabilitację, 153 chorych (51,5%) zdyskwalifikowano z niej. Chorzy skierowani na rehabilitację byli młodsi (63,5 ±9.3 vs. 69,7 ±10,7 lat, p <0,001), częściej stwierdzono u nich zawał STEMI (44,3% vs. 27,8%, p = 0,005) i rzadziej obciążeni byli nadciśnieniem tętniczym (65,7% vs. 76,7%, p = 0,04), w porównaniu do chorych zdyskwalifikowanych z rehabilitacji. Istotnie częściej palili tytoń (46,8% vs. 28,6%, p <0,01). Ocena laboratoryjna podczas wizyty wstępnej wKOS‑ZAWAŁ wykazała u pacjentów kierowanych na rehabilitację wyższe wyjściowe stężenia cholesterolu LDL oraz hemoglobiny, jak również niższe stężenia NT‑proBNP i kreatyniny w stosunku do pozostałych pacjentów (Tabela 1). Frakcja wyrzutowa lewej komory (LVEF) oceniona przy wypisie z oddziału była istotnie wyższa u pacjentów biorących udział w rehabilitacji (49,6 ±10,1% vs. 45 ±11,9%, p <0,01), rycina 1. Z kolei u pacjentów nieuczestniczących w rehabilitacji znacznie częściej zaobserwowano LVEF poniżej 40% (16,7% vs. 36,6%, p <0.01). W grupie chorych skierowanych na rehabilitację było więcej kobiet, jednak bez istotności statystycznej (53,8% vs. 45,6%, p = 0,183). WNIOSKI Prawie połowa pacjentów została skierowana na rehabilitację kardiologiczną. Byli to istotnie częściej pacjenci młodsi, po zawale STEMI, z korzystniejszymi wynikami badań laboratoryjnych oraz wyższą LVEF. Pacjenci starsi i bardziej obciążeni chorobami współistniejącymi rzadziej kierowani są na rehabilitację w programie KOS‑ZAWAŁ.
