- Abstract
Transformation of indolent follicular lymphoma (FL) designates its evolution into a more aggressive type of lymphoproliferation (in most cases diffuse large B-cell lymphoma) and represents a serious problem because together with therapy resistance it is the most frequent cause of death due to FL. Transformation is confirmed above all by the histological examination of involved tissue. Histological transformation may be suspected when indirect clinical and laboratory signs are present (hypercalcaemia, rapid lactate dehydrogenase elevation, rapid tumour growth, atypical or extranodal involvement, new intensive B-symptomatology). The reported incidence of transformation ranges from 10 to 70%. Compared with de-novo transformed follicular lymphoma, patients with transformation of follicular lymphoma during therapy have a poor prognosis. The tumour is characterized by rapid progression, therapy resistance and the majority of patients die within one year.
