Retalho de pericrânio como alternativa durante obliteração do ducto nasofrontal e seio frontal: relato de dois casos.

- Abstract
A região do seio frontal é acometida por traumas com menor frequência do que demais ossos da face, porém, por apresentar proximidade com estruturas nobres, como cérebro e olhos, este tipo de trauma se torna passível de estar relacionado a complicações pós-operatórias. Pode haver acometimento do seio frontal na parede anterior, posterior ou ambas, de modo que, o envolvimento da parede posterior torna a resolução dificultada, sendo geralmente necessário se pensar na obliteração do seio frontal e do ducto nasofrontal. Nestes casos, o retalho de pericrânio tem sido considerado uma boa alternativa para o tratamento, devido à sua disponibilidade e vascularização abundante. Objetivo: O presente trabalho visa relatar dois casos clínicos em que foi utilizado retalho de pericrânio para a obliteração do ducto nasofrontal. Conduta Clínica: Primeiramente os pacientes passaram por avaliação neurológica para posterior encaminhamento à equipe de Cirurgia e Traumatologia Buco-Maxilo- Facial para tratamento da fratura do seio frontal envolvendo parede anterior e posterior. Foi observado o comprometimento da patência do ducto nasofrontal em ambos os casos, optando por obliterá-lo, assim como ao seio frontal, utilizando retalho de pericrânio. Ademais, foi feita a reconstrução da parede anterior do seio frontal, por meio de sistemas de fixação com perfil 1,5. Resultados: Ainda que haja altas chances de complicações, os pacientes não apresentaram nenhuma queixa durante acompanhamento no período de um ano. O retalho de pericrânio mostrou-se como boa alternativa na necessidade de obliteração do seio frontal e ducto nasofrontal, por ser um tecido endógeno, com vascularização abundante e poucas complicações. Conclusão: Deste modo, de acordo com os casos apresentados, observou-se que o uso do retalho de pericrânio garantiu o sucesso do tratamento, sem complicações a curto e a longo prazo.
