CASE WITH DIFFUSE VENOUS THROMBOEMBOLISM: NEPHROTIC SYNDROME (MEMBRANOPROLIFERATIVE GLOMERULO NEPHRITIS) ASSOCIATED WITH FACTOR V LEIDEN MUTATION.

- Abstract
INTRODUCTION: Nephrotic syndrome characterized by hypoalbuminemia and hyperlipidemia is associated with an increased incidence of thromboembolism and increased platelet hyperaggregability. Factor V Leiden results in a hypercoagulable state by both increasing coagulation and decreasing anticoagulation.A case presented with multiple venous thrombosis due to nephrotic syndrome (membranoproliferative glomerulonephritis) associated with heterozygote factor V Leiden mutation. CASE: A 21-year-old male complained about stomachache at emergency service. On physical examination, bibasiller chest auscultation sounds declined, widespread venous collaterals on abdomen and bilateral pretibial edema ++++. The laborotary findings; hyperlipidemia(Cholestrol:302 mg/dl, LDL:183 mg/dl), hypoalbuminemia(1.9 g/dl), nephrotic range proteinuria(13.4 gr/day). In high resolution CT angiogram showed multiple thrombosis in different parts of venous system.Renal biopsy was performed. Renal biopsy pathology was reported:membranoproliferative glomerulonephritis. For the etiology of thrombophilia, the laboratory tests were performed and heterozygote factor V Leiden mutation was obtained. After he used cyclosporin and steroid theraphy for six months, partial remission was obtained. DISCUSSION: In nephrotic syndrome patients ,platelet aggregation ability increases because of the coagulation factors levels exchange. The FV Leiden mutation is the most common genetic risk for venous thrombosis and in selected populations with deep thrombosis or pulmonary embolism. Like our case , patients who had heterozygote FV mutation and condition of thrombosis risc factors like nephrotic syndrome was shown to become diffuse venous thromboembolism. CONCLUSION:In a young patient with diffuse venous thromboembolism and pulmonary embolism, FV leiden mutation associated with nephrotic syndrome must be considered in the differential diagnosis.
