- Abstract
Heart failure is a highly prevalent condition that involves a high number of consultations, frequent admissions and a high mortality. Proper management of these patients requires more knowledge of their treatment. Objectives. To identify the competences and the degree of autonomy of nurses in Heart Failure in Spain. Methods. Descriptive, cross-sectional, multicenter, national study, carried out using a self-administered questionnaire of 127 Items on nursing characteristics, workforce and role in Heart Failure units. Results. 84 centers are studied, out of which 70% have a Heart Failure Unit (n = 60). In the map by provinces of those centers having a Unit according to functional dependency they are all public except 5. In Spain the average is 1.29 units / million of inhabitants. The classification per rank is: 85% Unit, 13.3% Section, 1.7% Service. Regarding the type of Unit, 45% are a Specialized Unit and 35% are an Advanced heart failure Unit. The percentage of Units accredited by SEC is 41.7%. Regarding the specific training of nurses working in the Units, 26.7% have a specific Master degree and 3.3% possess a doctorate. 78.3% have an outpatient hospital. 85% have a specific nursing consultation. The specific interventions performed by the nurse are: pre-discharge visit to the patient 66.7%, planning of hospital-tohome transition 60%, telephone service 92.3%, drug titration according to medical prescription 75% and adjustment of diuretics 80.8%. Conclusions. An ongoing increase in Heart Failure Units accredited by SEC, with multidisciplinary teams, is appreciated, where the nurse specialized in heart failure has a fundamental role in the development of patient care and coordination programs.
