Terapia endodôntica conservadora e cirúrgica no tratamento de dens in dente: relato de dois casos.

- Abstract
Introdução: Dens in dente (DID) consiste em uma anomalia do desenvolvimento dentário, afetando mais os incisivos laterais superiores, tendo etiologia incerta. Devido a essa morfologia, possui maior risco de doenças pulpares, e o tratamento endodôntico pode ser complexo, sendo necessário o uso de métodos complementares. Objetivo: Relatar dois casos de tratamento endodôntico em incisivos laterais superiores com invaginações, de forma conservadora e cirúrgica. Relato de caso: No caso 1, a paciente possuía o dente #12 com diagnóstico de DID tipo II de Oehlers. Foi realizado o atendimento, e houve drenagem espontânea. No tratamento foram utilizadas pontas de ultrassom para remoção da invaginação. Após solicitação de tomografia computadorizada de feixe cônico (TCFC), foi planejada abordagem cirúrgica, que consistiu em retirada e curetagem de lesão. No caso 2, a paciente possuía o dente #22 com diagnóstico de DID tipo III de Oehlers. No tratamento, também foi solicitada a TCFC, foram utilizadas pontas de ultrassom e magnificação com microscópio, para evitar o desgaste excessivo de dentina, bem como a realização de um tampão apical com MTA e obturação com cimento biocerâmico. Após acompanhamento, as pacientes encontram-se assintomáticas. Conclusão: Devido à morfologia diversificada do DID, é necessário conhecimento da anatomia dentária pelos profissionais, além de tecnologias auxiliares como a TCFC, pontas de ultrassom, microscópio cirúrgico e obturação termoplastificada com cimentos biocerâmico, para que haja um plano de tratamento eficiente que permita a resolução da queixa do paciente.
