The article reflects on the conflict between treatment optimization and registration of radiopharmaceuticals with fixed activity posology in oncological nuclear medicine therapy. Topics include maximum tolerable absorbed dose approach in nuclear medicine therapy; posology in current radiopharmaceutical registrations; and optimization principle in radioembolization procedures.
