¿La COVID-19 agrava a los pacientes con enfermedades neuropsiquiátricas previas?

- Abstract
Objetivo: evaluar la agravación subjetiva de las enfermedades neurológicas/psiquiátricas en el contexto de la pandemia de SARS-Cov-2. Antecedentes: la patogenia originada por el SARSCov2 puede ser relacionada por la reacción inflamatoria periférica desencadenada. Como consecuencia de esta última, un fenómeno neuroinflamatorio podría originarse con la posible agravación de patologías neuropsiquiátricas previas. Así mismo, otros factores estresores podrían estar involucrados. Métodos: incluimos 3 grupos de pacientes del INNN: 89 pacientes que desarrollaron COVID-19 sintomático, 40 pacientes que desarrollaron COVID-19 asintomático, y un grupo control (47 pacientes sin antecedente de COVID y seronegativo). Se les preguntó sobre la severidad del cuadro COVID-19 y sobre la evolución de su cuadro neuropsiquiátrico previo. También se evaluaron en suero diferentes citocinas pro y antiinflamatorias. Núm. de registro del protocolo: 32/20 Resultados: el 30,7% de los individuos incluidos consideraron que su patología basal había empeorado durante el periodo de estudio. Esta sensación fue significativamente más frecuente (p=0,01) en los pacientes con COVID-19 sintomático (39,3%) que en los pacientes de los otros 2 grupos (21,8%). El empeoramiento no estaba relacionado con la gravedad de la COVID-19. Comparando los pacientes con enfermedad estable vs. los pacientes que se empeoraron, la IL-10 fue significativamente más alta en este último grupo (p= 0.008) Conclusiones: estos resultados confirman el impacto de la pandemia de SARS-Cov-2 en los pacientes con enfermedades neurológicas / psiquiátricas. El confinamiento, la falta de atención médica y el estrés son factores que contribuyen a ello. Aunque el hallazgo de una mayor frecuencia de empeoramiento en los pacientes sintomáticos de COVID-19 puede estar relacionado con una mayor ansiedad/depresión en este grupo, no podemos excluir el involucramiento del fenómeno inflamatorio. En este sentido, el aumento significativo de la IL-10 en este grupo podría estar relacionada con una respuesta inmunomoduladora tardía, en respuesta a un incremento de la inflamación.
