- Abstract
Introducción: La influencia de las madres en el desarrollo a lo largo de la vida de sus hijos es fundamental, la madre define y percibe los acontecimientos teniendo en cuenta el conocimiento adquirido a través de la socialización en su vida, en donde el entorno, el apoyo de la familia y los amigos juegan un papel muy importante para hacer a la mujer capaz de adoptar el papel de madre y saber cómo cuidar a su hijo, es decir la madre asume su rol de manera particular independiente del sistema social (Mércer, 1985). Esto implica, bajo la teoría de adopción del rol maternal, la necesidad de tener en cuenta el entorno familiar, la escolaridad, el trabajo, las creencias religiosas y otras entidades de la comunidad como elementos importantes en la adopción de este rol, en donde la madre se va sintiendo vinculada a su hijo, adquiere competencia en la realización de los cuidados asociados a su rol y experimenta placer y gratificación dentro del mismo (Alvarado, Guarín, & Cañón-Montañéz, 2011). En esta línea, fundamental tener presentes factores como: edad, estado de salud, función de la familia, relación padre-madre y características del lactante (en este caso por ser un niño prematuro y de bajo peso al nacer) y el apoyo de la pareja como factor predisponente para hacer posible el cuidado del hijo (Bee, Legge, & Meighan, 1999). A partir de las vivencias de las gestantes hay algunos elementos que pueden contribuir a mejorar la prestación de los servicios de salud, no todas las embarazadas reciben la noticia del embarazo de la misma manera y pueden experimentar emociones diversas que dependen de factores internos (experiencias previas, personalidad, fortalezas, etc.) y de factores externos como el apoyo familiar, las condiciones socioeconómicas y la calidad de las relaciones con su pareja (Carnaval, González, & Tovar, 1998). En ese sentido, Barnard (2000; citada por Shonkff & Meisels, 2000) desarrolló el modelo de interacción para la valoración de la salud infantil (Child Health Assesment Interaction Model). Su modelo y su teoría fueron el resultado del proyecto de valoración de Enfermería infantil. Barnard cree que el sistema padres-lactante está influido por las características de cada uno de los miembros. Estas características se modifican para satisfacer las necesidades del sistema mediante un comportamiento adaptativo. La interacción entre el padre (o el cuidador) y el niño se describe en el modelo de Barnard mediante cinco señales y actividades: 1) la claridad del lactante en la emisión de señales; 2) la reactividad del lactante a los padres; 3) la sensibilidad de los padres a las señales del lactante; 4) la capacidad de los padres para identificar y aliviar la incomodidad del lactante y 5) las actividades sociales, emocionales y cognitivas estimulantes del desarrollo desplegadas por los padres. Un aspecto importante en las afirmaciones teóricas de Barnard es que la enfermera proporciona sobre todo apoyo a la sensibilidad y a la respuesta de la madre a los signos de su hijo, en lugar de intentar cambiar sus características y estilo de cuidados maternales. La madre y padre al enfrentarse a la prematurez de su hijo recién nacido se encuentra ante una situación adversa donde es difícil desarrollar su rol innato de madre y cuidadora y por ende se hace más difícil la percepción de las necesidades del lactante. Es aquí donde el Programa Madre Canguro se convierte en un apoyo fundamental, donde la educación a los padres y cuidadores, la supervisión permanente de todas las señales de salud y enfermedad que expresa el prematuro transcienden y posibilitan la calidad de vida de la familia lactante. Molina (1992), afirma que el Programa Madre Canguro surgió en Colombia como respuesta a una situación crítica de hacinamiento, falta de tecnología, infecciones cruzadas nosocomiales, pobre pronóstico, ausencia de recursos de alta tecnología y una mortalidad extremadamente alta para niños con bajo peso en las unidades de neonatología durante los años 70. Charpak y Figueroa (1996), en un recuento histórico del Programa Madre Canguro en Bogotá, Colombia, en el Instituto Materno Infantil (IMI), la maternidad pública más grande del país, 20,000 partos por año en esta época, el Dr. Edgar Rey Sanabria, entonces jefe del servicio de neonatología y profesor de pediatría de la Universidad Nacional de Colombia, decidió utilizar a las madres como incubadoras ante la insuficiente cantidad de estas, instituyendo lo que él llamó técnica Madre Canguro, en todos los recién nacidos "sanos", con peso al nacer <2,000g. En Bucaramanga, el Programa "Madre Canguro" en el Hospital Universitario de Santander (HUS), se ha convertido en una alternativa eficaz y segura para brindar atención integral a los neonatos prematuros (menores de 37 semanas de edad gestacional) y/o de bajo peso al nacer (menores de 2,500 gramos), y evitar posibles complicaciones y secuelas que traerían dichas situaciones, desde el punto de vista médico, psicológico y social. http://hus.gov.co/index. Evaluar el efecto de Objetivo: l Programa Madre Canguro en las áreas del desarrollo psicológico (locomotora, personal, social, audición y lenguaje, y coordinación óculo manual, agudeza sensoperceptual, constancia de objetos, memoria, aprendizaje, capacidad para resolver problemas, generalización y clasificación) y en las medidas antropométricas de los niños (talla, peso, perímetro cefálico) y fisiológicas (temperatura, frecuencia cardiaca, frecuencia respiratoria, patrón de alimentación), en niños prematuros (28 a 36 semanas de edad postconcepcional, peso menor a 2,500gr al nacer). Metodología: Se realizó investigación de tipo cohorte, se escogieron dos grupos de niños: un grupo experimental (E), los niños que nacen con bajo peso y prematuros entre las 32 a las 36 semanas de edad gestacional, que participan del Programa Madre Canguro y otro grupo control (C), conformado por niños nacidos entre las 32 a las 36 semanas de edad gestacional y que no estuvieron expuestos al programa. Teniendo en cuenta que el estudio se realizó en los niños que ingresaron al Programa Madre Canguro del Hospital Universitario de Santander (HUS), la población corresponde al total de niños que nacen entre las 32 a 36 semanas de edad gestacional y estuvieron en condiciones clínicas y cumplieron los criterios de inclusión para formar parte de cada grupo control y experimental. Se contemplaron los siguientes criterios de inclusión de la población: niños con edad gestacional de 32 a 36 semanas, niños de bajo peso al nacer menor de 2,500 gr, niños seleccionados con el criterio establecido para el Programa Madre Canguro del Hospital Universitario de Santander. Y como criterios de exclusión: niños que no cumplían con la edad gestacional de 32 a 36 semanas, niños con severidad clínica, niños que interrumpieron su asistencia al Programa Madre Canguro y niños del grupo control que se encontraban en programas de estimulación temprana después de las 40 semanas corregidas (durante los primeros 18 meses de edad). Se empleó como primera prueba psicológica el Test de Desarrollo Infantil Griffiths, creado por John Griffiths en 1954 en Inglaterra, revisada y completada en 1964 por Ruth Griffiths. Se distingue por ser un instrumento de buena confiabilidad y validez discriminativa. Buen valor predictivo del desarrollo a 4, 8, y 12 años. Ha sido estandarizada a la población colombiana en 1977 y 1985 por Vuori y Ortiz y ha sido utilizado por 20 años en los Programas Madre Canguro con niños Pretérmino y a término. Como segundo instrumento psicológico, se empleó la Escala Comportamental Bayley, fue diseñada y validada por Nancy Bayley de la Universidad de Berckeley en el año 1969 para evaluar niños con edades comprendidas entre el nacimiento y los 30 meses. Posteriormente se revisa en el año 1984 y por último en 1993, cuando se extendió su rango de aplicación hasta los 42 meses. Se utilizó además la prueba Home Inventory, para la medición de la calidad de estimulación y apoyo dado a un niño en su hogar. Esta prueba refiere que la calidad del entorno en el hogar, en referencia a la calidad de la estimulación y a las oportunidades de aprendizaje, está estrechamente ligada al bienestar del niño. La prueba fue desarrollada en 1984 por Caldwell y Bradley. Resultados: Los resultados muestran que las familias pertenecen en su mayoría al estrato 1 y 2, con 2 ó 3 servicios públicos y régimen subsidiado en salud. Algunos niños están expuestos a ambientes contaminantes o hacinamiento en sus viviendas. Se resalta que los factores ambientales referidos a los adultos cuidadores ejercen influencia en el desarrollo cuando se anticipan a las necesidades de los niños y no estimulan el lenguaje, la comunicación y la lectura y tampoco ofrecen juguetes para estimular la cognición. En los dos grupos (experimental y control), entre los 12 y los 18 meses, la alimentación ofrecida a los niños no contiene los nutrientes necesarios para sus necesidades de crecimiento; sin embargo poseen talla, peso y perímetro cefálico acorde con el estándar de la Organización Mundial de la Salud. Todas las madres de los grupos asistieron a control prenatal pero no se evidencian acciones preventivas para la prematurez en referencia al cuidado psicológico. Hay diferencias en la reacción al extraño en los niños a la edad de 12 meses, los niños del programa son sociables y no evitan al evaluador, mientras que en el grupo control presentan reacción al extraño esperada con la edad. A la edad de 18 meses los niños de los dos grupos presentan reacción al extraño pero con mayor intensidad de respuesta en algunos niños del grupo experimental. Se reportan 9 niños muertos, uno con hemiparesia e hipotonía muscular en el grupo control, hecho que no ocurre en el grupo experimental, por lo cual se evidencia que el Programa Madre Canguro ofrece la oportunidad de garantizar la supervivencia a los niños y niñas en el primer año y prevenir riesgos futuros. En cuanto al efecto del Programa Madre Canguro en el desarrollo de los niños y niñas, tanto del grupo control como del grupo experimental, se pudo evidenciar en los resultados que en la evaluación médica, medidas antropométricas y psicológicas, no se encuentran hallazgos clínicos, ni patológicos que indiquen diferencias marcadas entre ellos. En el grupo control a diferencia del grupo experimental, se encontró que las madres mostraron menos atención en la higiene de sus hijos, observado en la ropa, piel y cabello sucio (por tierra de su tipo vivienda, manchas de alimentos, entre otras). En los dos grupos se encontró que las condiciones ambientales en las que viven ayudan a la proliferación de zancudos, razón por la cual los niños presentaron lesiones notorias en la piel. En otras situaciones, sin discriminación entre grupo control ni experimental, las condiciones físicas de la vivienda ofrecen riesgo de accidentalidad. Se evidenció además que las madres adoptan su papel materno independientemente del acompañamiento que tuvieron en este proceso, no encontrando diferencias entre lo que perciben de sus hijos y las condiciones de apoyo en su proceso de crecimiento y desarrollo, y los patrones de comportamiento con sus hijos no variaron si ellas estuvieron en el Programa Madre Canguro o no. El estado emocional de los progenitores y cuidadores, unido a las pobres condiciones físicas del ambiente, a la agresividad del entorno, a las limitadas oportunidades de superación de estas barreras que son las mismas para ambos grupos de estudio, son en últimas las razones para que el avance en el desarrollo físico, intelectual, emocional y psicológico de los prematuros y nacidos de bajo peso, que logran sobrevir, tengan similares resultados. Conclusiones: En la investigación se evaluó el efecto del Programa Madre Canguro desde la perspectiva interdisciplinaria, integrando los profesionales de la salud en Medicina, Enfermería y Psicología, lo que permite involucrar un elemento innovador dentro del estudio. Dentro de las bondades del Programa Madre Canguro se considera la perspectiva psicológica de las madres, en referencia a la problemática de prematurez de su hijo, abordaje con cuidado integral y holístico de la familia gestante y lactante, el fortalecimiento de la red de apoyo social y del grupo familiar. Las atenciones desde el Programa Madre Canguro están dirigidas al cuidado del bebé y al éxito en la adherencia al tratamiento. Independiente de la participación del Programa Madre Canguro, los padres se preocupan a su modo por la salud y el bienestar de sus hijos. Es importante destacar que el Programa Madre Canguro es exitoso al permitir la detección precoz de eventos con riesgo de secuelas, lo que permite actuar rápidamente con acciones hacia los padres y los niños, pero no es objeto del programa cambiar las circunstancias sociales, económicas, culturales, políticas en las que se desenvuelven estos grupos poblacionales. Teniendo en cuenta lo expuesto hasta el momento, se derivan algunos interrogantes que pueden orientar investigaciones posteriores. ¿Qué impacto tienen los controles prenatales en la prevención del parto prematuro?; sería muy interesante hacer un estudio poblacional del número de prematuros en los distintos estratos socioeconómicos de la ciudad para evaluar si la condición socioeconómica de las madres es un factor determinante de la prematurez; ¿cuáles son los imaginarios sociales de las madres o cuidadores de los niños y niñas respecto a la alimentación, la higiene y desarrollo?; ¿cuáles son los imaginarios sociales de los profesionales de la salud en relación al prematuro?; ¿el conocimiento que poseen los profesionales de la salud sobre la gestante concuerda con sus representaciones sociales?; ¿cuáles son los imaginarios sociales sobre la autonomía de los niños y niñas menores de dos (2) años?; ¿cómo favorecer la visión holística de la salud en los profesionales que atienden a las madres con hijos prematuros?; ¿cuáles son los rasgos de personalidad de los padres y cuidadores que influyen en la adherencia a tratamientos de salud?
