- Abstract
WSTĘP Niewydolność serca istotnie zmniejsza wydolność fizyczną oraz negatywnie oddziałuje na cały organizm. W skrajnych przypadkach niewydolności serca jedynym sposobem leczenia przyczynowego jest leczenie operacyjne polegające na przeszczepieniu serca. Z powodu niewystarczającej liczby organów do przeszczepu, coraz częściej stosuje się urządzenia do mechanicznego wspomagania pracy komór serca (VAD). Rehabilitacja pacjentów z VAD jest bardzo ważnym elementem terapii ze względu na długotrwałe unieruchomienie chorego, oczekującego na przeszczep lub regenerację serca. CEL Celem badania była ocena wpływu wczesnej poszpitalnej rehabilitacji z kontynuacją treningów w domu pod nadzorem telemedycznym na tolerancję wysiłku, siłę mięśniową i koordynację złożoną u chorych po implantacji LVAD. METODY Do badania włączono 20 mężczyzn w wieku 19–66 lat powszczepieniu LVAD. Wszyscy podlegali standardowej rehabilitacji na oddziale pooperacyjnym izachowawczym. Po zakończeniu leczenia szpitalnego pacjenci rozpoczęli treningi w klinice rehabilitacji (4–5 tygodni): treningi wytrzymałościowe, ćwiczenia ogólnousprawniające z elementami ćwiczeń równoważnych i koordynacyjnych, elementy ćwiczeń oporowych. Uwszystkich przed rozpoczęciem rehabilitacji i po jej zakończeniu wykonano: ergospirometrię, 6 minutowy test korytarzowy, zbadano siłę uścisku prawej i lewej ręki, siłę mięśni kończyn dolnych oraz koordynację złożoną (test up&go). WYNIKI Zaobserwowano istotny wzrost wartości większości badanych parametrów po zakończeniu rehabilitacji stacjonarnej (1 przed rehabilitacją vs 2 po rehabilitacji): VO2 peak [ml/kg/min]: 1,1 ±2,2 vs 12,5 ±2,7, p <0,001; Watt: 42,6 ±12,4 vs 54,1 ±13,1, p <0,0001; 6 MWT [m]: 300,1 ±102,2 vs 404,8 ±105,9, p <0,0001; 30 Second Chair Stand [il. powtórzeń] 8,4 ±3,3 vs 11,6 ±4,8, p <0,0001; up&go [sek] 9,0 ±1,7 vs 7,1 ±1,5, p <0,0001; siła uścisku lewej ręki [kg]: 31,5 ±8,4 vs 34,8 ±8,1, ns; siła uścisku prawej ręki [kg]: 33,6 ±11,2 vs 36,0 ±9,0, ns. WNIOSKI 1. Rehabilitacja przynosi istotne korzyści wzakresie tolerancji wysiłku, siły mięśniowej izłożonej koordynacji pacjentom po implantacji LVAD. 2. Rehabilitacja jest bezpieczna i skuteczna w tej grupie chorych.
