- Abstract
Os adenocarcinomas intestinais são tumores localmente invasivos com moderada taxa metastática para linfonodos regionais, peritônio, omento e fígado, e eventualmente para o pulmão, baço, ovários, testículos e rins. O diagnóstico geralmente ocorre tardiamente porque os sinais clínicos são inespecíficos, como vômito, anorexia, dor abdominal, perda de peso, melena e diarreia (ROMANO et al. 2019). A radiografia abdominal pode revelar uma massa intestinal e evidência de obstrução. Os achados ultrassonográficos comuns incluem espessamento intestinal, perda de estratificação da parede intestinal, diminuição da motilidade e linfadenopatia regional (SMITH et al., 2019). Um canino da raça Pinscher, fêmea, com 9 anos de idade apresentou inicialmente, episódios esporádicos de vômito e diarreia, que evoluíram para gastroenterite hemorrágica crônica. Os exames de imagem não revelaram alterações conclusivas. Após sucessivas tentativas de diagnóstico e terapias sem sucesso, optou-se pela laparotomia exploratória, sendo observado baço com padrão em noz moscada e espessamento da porção duodenal descendente do intestino delgado. Foram coletados fragmentos desses órgãos para avaliação histopatológica. A mucosa intestinal exibia proliferação multifocal com áreas de coalescência de células epiteliais organizadas em ilhas e trabéculas com padrão infiltrativo em submucosa e muscular da mucosa, exibindo acentuado pleomorfismo e atipias como anisocitose e anisocariose acentuadas, núcleos vesiculares, nucléolos evidentes, macronucleolose, cariomegalia, binucleações e presença acentuada de figuras de mitose atípicas. Presença de êmbolos oncóticos linfáticos multifocais em submucosa e infiltração focal em serosa. O baço exibia proliferação de células epiteliais neoplásicas com características semelhantes às observadas no intestino. Parênquima esplênico apresentando rarefação de folículos linfóides e congestão. Presença focalmente extensa de trombo arterial associado a células neoplásicas. Com base nesse resultado, foi estabelecido o diagnóstico de adenocarcinoma intestinal de alto grau com metástase, com prognóstico desfavorável. O animal foi a óbito uma semana após a cirurgia.
