- Abstract
Introducción. Las condiciones socioeconómicas de las poblaciones afectadas por la COVID-19 son un factor que determina las comorbilidades presentes en una población y se postulan como un importante factor de riesgo de complicaciones de salud relacionadas con la educación, la salubridad y el acceso a los servicios médicos. Objetivo. Determinar el impacto del nivel socioeconómico establecido por el Sistema de Identificación de Potenciales Beneficiarios de Programas Sociales (SISBEN) de Colombia en las comorbilidades, coinfecciones bacterianas y la mortalidad asociadas con la COVID-19. Materiales y métodos. Se hizo un estudio descriptivo observacional retrospectivo de pacientes en la unidad de cuidados intensivos con neumonía grave por la COVID-19 confirmad por PCR/antígeno en un centro hospitalario de tercer nivel de atención en Villavicencio. Se obtuvieron datos demográficos del SISBEN y las historias clínicas. Se clasificó a los pacientes según su categoría de clasificación, comparando comorbilidades, infecciones bacterianas y mortalidad. Resultados. Se incluyeron 235 pacientes con una edad media de 62,7 años (DE: 15,0); el 61,2 % correspondía a hombres. Los grupos en pobreza moderada y vulnerabilidad presentaron mayor prevalencia de enfermedades cardiovasculares e hipertensivas, así como una mayor frecuencia de superinfecciones bacterianas (54 %). El grupo poblacional sin clasificación de SISBEN presentó una mayor mortalidad (59 %) y porcentaje de coinfecciones bacterianas (22,5 %). El promedio en el APACHE al ingreso a la unidad de cuidados intensicos (UCI) fue mayor en el grupo clasificado como ni pobre ni vulnerable, el cual ocupó el segundo lugar en la mortalidad. Conclusión. Las condiciones de vulnerabilidad, pobreza y bajo nivel socioeconómico se asociaron con una mayor prevalencia de enfermedades cardiovasculares y metabólicas y mayor incidencia de superinfecciones bacterianas en pacientes con COVID-19.
