Prolapso retal tipo II em um equino.

- Abstract
O prolapso retal, caracterizado pela eversão do reto através do orifício anal, é uma afecção pouco frequente em equinos. Quando surge, geralmente está relacionado às causas digestivas, apresentando um aumento da pressão intra-abominal, incluindo obstrução, diarreias, neoplasias, partos distorcicos, enemas, entre outros. Os prolapsos tipo II envolvem toda a parede retal, sendo que a porção ventral do tecido prolapso é mais espessa do que a porção dorsal. Quando existe necrose, geralmente faz-se a ressecção cirúrgica do coto prolapsado para posteriormente tratar a afeção primária. Devido à pouca literatura relatando a resolução dessa patologia na clínica, o objetivo do presente trabalho é apresentar o caso de um equino, fêmea, gestante de 6 meses, SRD, 11 anos, 285 kg, oriundo do município de Timon, Maranhão. Esse animal foi encaminhado à Clínica de Grandes Animais da UFPI, com queixa principal de aumento do volume na região anal. Na anamnese, o proprietário relatou que a égua demonstrou desconforto com fortes contrações do abdômen, cavando e rolando por dois dias consecutivos. No exame clínico, observouse tempratura corporal de 37,9 °C, frequência cardíaca de 54 bpm, frequência respiratória de 30 mpm, tempo de preenchimento capilar de 2", mucosas róseas, movimentos intestinais hipomotílicos em todos os quadrantes, presença de fezes com muco em poucas quantidades e ressecadas, presença de prolapso tipo II (13 x 15 x 10 cm) com áreas de congestão, edema e necrose, com presença de secreção serosanguinolenta, e apetite presente. Após exame clínico, introduziu-se uma sonda lubrificada entre a massa prolapsada, onde não houve resistência da sonda, em aproximadamente 20 cm. Tentou-se repor o reto manualmente, sem êxito. O hemograma revelou parâmetros dentro da normalidade. O tratamento estipulado foi à base de benzilpenicilina G procaína 20.000 UI/kg, benzilpenicilina G benzatina 20.000 UI/kg, sulfato de dihidroestreptomicina 21 mg/kg por via intramuscular (SID), durante cinco dias; flunixin meglumine 1,1 mg/kg por via intravenosa de SID, durante sete dias; soro antitetânico 5000 UI/k por via intramuscular, dose única, com reforço após sete dias; e adição de óleo mineral 220 ml, via oral. O tratamento tópico consistiu em enema com solução de óleo mineral 1:1 de água morna, durante 5 dias; limpeza com permanganato de potássio; dimetilsulfóxido (DMSO) 99,2%, 30 ml; e pomada nitrofurazona sobre a mucosa do reto. Após seis dias de tratamento clínico, houve regressão do prolapso, com o animal retornando a defecar normalmente e sem recidiva do prolapso. Após cinco dias em observação, o animal recebeu alta médica. Entre os fatores correlacionados aos quadros de prolapso retal em equinos, concorda-se com a literatura consultada, sugerindo que os prolapsos tipo II podem ser corrigidos na clínica após resolução da causa primária e eliminação da irritação e dor. O tratamento clínico estipulado foi efetivo para a égua levar a gestação a termo, com parto normal.
