Epilepsia farmacorresistente asociada a Neurocisticercosis calcificada en una población de pacientes del INNNMVS.

- Abstract
Objetivo: Establecer la asociación entre el número y localización de lesiones de neurocisticercosis (NCC) con la presencia de epilepsia fármacorresistente (FR) en pacientes con epilepsia por NCC calcificada atendidos en el INNMVS. Antecedentes: La epilepsia FR consiste en la persistencia de crisis tras el fracaso de dos fármacos antiepilépticos (FAEs) adecuados, bien tolerados, en monoterapia o combinación (1). La asociación de NCC con FR es poco común (2). La prevalencia de ER en el INNN se describió de hasta el 56%, incluyendo en esa muestra 2.4% de pacientes con NCC que persistían con crisis (3). Métodos: Se realizó un estudio transversal, analítico, retrospectivo, con pacientes atendidos en la clínica de epilepsia del INNN desde 2013 hasta 2019, mayores de 18 años con diagnóstico de epilepsia y neurocisticercosis calcificada. Se utilizó el programa SPSS v 18.0. El análisis cuantitativo fue con frecuencias y porcentajes y el cualitativo con medias y desviación estándar. Para las asociaciones se utilizó prueba de Chi cuadrada de Pearson o exacta de Fisher para variables cualitativas y prueba T de Student o U de Mann-Whitney para las variables cuantitativas. Núm. de registro del protocolo: 104/19. Resultados: Se incluyeron 41 pacientes. La edad media de los pacientes fue de 48 años ± 12.1, 24 (58.5 %) del sexo femenino, la edad promedio al inicio de las crisis fue a los 24.5 ± 16.2 años. En cuanto al número de lesiones de NCC se encontró en promedio 3.44± 6 por paciente. Trece pacientes (31.7%) estaban libres de crisis. Veintidós pacientes (53.7%) presentaban una lesión única, el resto dos o más. En relación con el número de NCC calcificados en imagen y libertad de crisis se encontró una p: 0.05 en donde los pacientes con menos lesiones cursaban con mejor control, 25 pacientes (61%) de la NCC se encontraba en el lóbulo temporal y 16 (39%) extratemporales, sin diferencias en cuanto a control de crisis (p:0.73), el promedio de FAEs usados fue 1.83, los pacientes con NCC temporal usaban un mayor número de FAES 2.04, vs 1.5 en los pacientes con NCC extratemporal (p:0.05). Conclusiones: La presencia de más lesiones calcificadas de NCC se asoció FR. La localización de la NCC en el lóbulo temporal no incremento la FR, sin embargo, si se asoció a un mayor uso de FAES. Al momento de iniciar tratamiento para epilepsia asociada a NCC hay que tener en cuenta la localización y el número de lesiones.
