The Effect of Angiotensin Receptor Blockers or Thiazolidinediones on Inflammatory Cytokines in Diabetic State.

- Abstract
Evidence has been accumulated that some of the angiotensin II type 1 receptor blockers have insulin- sensitizing property. Insulin resistance is the comer stone not only in the development of type 2 diabetes mellitus but also in the development of most of its vascular complications. Insulin resistance is associated with a chronic low grade inflammation. Proinflammatory cytokines are recently reported to inhibit insulin signaling causing insulin resistance. Recent studies indicate the role of TNF-α in the production of insulin resistance. In the current study we investigate the effect of telmisartan (a new ARB) or pioglitazone (one of thiazolidinediones) on the IL-6 and MCP-1 production induced by TNF-α in adipocytes. We stimulate 3T3-L1 adipocytes with TNF-α after treating the cells with telmisartan or pioglitazone. We measure the concentration of IL-6 and MCP-1 in the media by ELISA. We also investigate the phosphorylation of ERK and p38 MAP kinase by Western blot. There was a significant increase of IL-6 production after stimulation by TNF-α. By pretreatment with telmisartan we found a significant reduction in the production of IL-6. We also found that TNF-α stimulation increases the phosphorylation of both p38 MAPK and ERK, and by pretreatment with telmisartan the ERK phosphorylation is decreased. By pretreatment with pioglitazone we found a significant reduction in the production of MCP-1 and in the phosphorylation of p38 MAPK. We found here that pretreatment with telmisartan IL-6 production induced by TNF-α from adipocytes is decreased probably due to the inhibition of ERK kinase activation, and pretreatment with pioglitazone MCP-1 production induced by TNF-α from adipocytes is decreased probably due to the inhibition of p38 MAP kinase activation. We suggest that telmisartan may improve insulin resistance in diabetic hypertensive patients by inhibition of IL-6 production from adipocytes.
