- Abstract
Objetivo: Se relacionaron las disminuciones de la cordancia prefrontal en el electroencefalograma (EEG), que se detectan ya a las 48 horas de la instauración de la medicación, con los resultados clínicos de los ensayos terapéuticos del trastorno depresivo mayor. Se desconoce la relación entre los cambios cerebrales durante la fase de introducción con placebo y la respuesta farmacoterapéutica. Los autores plantearon la hipótesis de que las disminuciones de la cordancia prefrontal durante la fase de introducción con placebo se relacionarían con una respuesta clínica más favorable en los individuos tratados con antidepresivos. Método: Se reagruparon los datos de 51 adultos con depresión mayor procedentes de dos ensayos independientes, doble ciego, controlados por placebo. La face de introducción con placebo a simple ciego, de 1 semana, dio paso a 8 semanas de tratamiento aleatorizado con fármacos (20 mg de fluoxetina o 150 mg de venlafaxina) o placebo. Los autores obtuvieron las medidas cuantitativas de la cordancia en el EEG en la evaluación inicial y al final del período de introducción con placebo. Se examinaron las relaciones entre los cambios de la cordancia regional al final del período de introducción con placebo y la respuesta clínica (las puntuaciones finales de la Escala de Hamilton para la Depresión de 17 ítems) utilizando análisis de regresión lineal múltiple. Resultados: Como se había postulado, las disminuciones de la cordancia prefrontal durante la fase de introducción con placebo se relacionaron con unas puntuaciones finales más bajas en la Escala de Hamilton para la Depresión en los individuos que se habían asignado de forma aleatorizada al tratamiento farmacológico. Los cambios prefrontales explicaban el 19 % de la varianza en los resultados finales de la Escala de Hamilton para la Depresión. Conclusiones: Los cambios neurofisiológicos que se producen durante la fase de introducción con placebo podrían servir de biomarcadores no farmacodinámicos de la futura respuesta terapéutica en los ensayos clínicos de la depresión mayor.
