- Abstract
Objetivo: Relatar a observação de sangramento intestinal por cintilografia de hemácias marcadas. O sangramento intestinal baixo representa cerca de 25 a 33% dos casos, sendo menos comum que o alto, e pode atingir uma mortalidade de até 4%. Descrição do caso: Paciente E.M.A, masculino, 88 anos, internado na enfermaria do Hospital das Forças Armadas para investigação de melena e queda de hematócrito, que chegou a 10%. Apresenta história de acidente vascular cerebral prévio, sendo um há 4 anos e outro há um ano, e 12 pontos na escala de Glasgow. Realizou endoscopia digestiva alta que não evidenciou sangramento gástrico. Realizada então Cintilografia com hemácias marcadas, sem evidências de sangramento intestinal nas imagens até 2 horas após a administração de tecnécio 99m-pirofosfato (método de marcação in vivo). Nas imagens tardias, após 12 horas da administração do radiofármaco, revelou-se a presença de sangramento intestinal no colón transverso, com progressão para o colón descendente. Discussão: Os pacientes apresentam-se geralmente assintomáticos até que o volume de sangramento ultrapasse 100mL/dia. A Medicina Nuclear dispõe de duas técnicas para investigação de sangramento intestinal: Cintilografia com hemácias marcadas e Cintilografia com Colóide. A Cintilografia com hemácias marcadas in vivo é realizada mediante injeção pirofosfato, seguida por Tecnécio 99m. Indicada para pesquisa de sangramentos intestinais intermitentes e de baixo volume (0,1-0,5 ml/min), com sensibilidade de 93% e especificidade de 95%. A Cintilografia com Estanho coloidal 99mTc é uma alternativa, sendo sensível para sangramentos ativos de até 0,05-0,1 ml/min; no entanto, apresenta rápido clearence, o que impossibilita aquisição de imagens seriadas e tardias. Já a Angiografia, requer sangramento da monta de 1,5-3,0 ml/min para ser detectado, e localiza cerca de 65% dos sítios quando a taxa excede 1ml/min. Conclusão: Causas de sangramento são variadas e podem ser manifestação clínica de angiodisplasias, doença diverticular, neoplasia ou áreas de isquemia. A pesquisa de sangramento digestivo ativo por cintilografia é útil também para guiar a Angiografia. Os dois radiofármacos citados podem ser utilizados para tal pesquisa, cada um com suas particularidades, e sendo, de forma geral, cerca de 10 vezes mais sensíveis que a Angiografia na detecção dos sítios de sangramento intestinal.
