- Abstract
Zusammenfassung Hintergrund Die abdominoperineale Rektumamputation ist das Verfahren der Wahl bei tief sitzenden Rektumkarzinomen, wenn kein Sphinktererhalt möglich ist. Neben Komplikationen wie Impotenz, Blasenfunktions- und Wundheilungsstörungen ist die perineale Hernie eine sehr seltene Komplikation dieser Operationsmethode. Die operative Versorgung kann sowohl abdominal-transperitoneal mittels Einnaht eines Kunststoffnetzes erfolgen als auch von perineal. Wir berichten über den 3. Fall einer perinealen Hernie in der deutschsprachigen Literatur. Fallbericht Bei einem 66-jährigen Patienten kam es 2 Jahre nach abdominoperinealer Rektumamputation und adjuvanter Chemotherapie bei einem T2N0M0-Adenokarzinom des Rektums zu einer perinealen Hernie. Die Diagnose wurde mittels Magnetresonanztomographie bestätig und die operative Versorgung erfolgte offen chirurgisch durch einen perinealen Zugang mittels Implantation eines Kunststoffnetzes. Diskussion Die operative Versorgung einer perinealen Hernie kann sowohl transabdominal als auch perineal oder als Kombinationseingriff erfolgen. Der transabdominelle Zugangsweg scheint vorteilhaft gegenüber dem perinealen, allerdings kann bei kleineren perinealen Hernien der perineale Zugang Methode der Wahl sein. Da es sich im hier vorgestellten Fall um eine unkomplizierte Hernie handelte, haben wir den wesentlich atraumatischeren Weg des perinealen Zugangs bei der operativen Versorgung der Hernie gewählt.
