Death causes analysis of pregnant and lying-in women death cases in Haikou, 2010-2020.

- Abstract
Objective To explore the causes of maternal deaths in Haikou City, analyze the review of maternal deaths, and we provide a theoretical basis for effective reduction of maternal mortality. Methods The monitoring reports of maternal deaths in Haikou City from 2010 to 2020 were collected, and the data of maternal death cases were reviewed according to the twelve grid method, and the distribution of maternal deaths in Haikou City, the conclusions of death review and related factors affecting death were analyzed. Results From 2010 to 2020, the number of live births with local household registration in Haikou City was 232 088, 31 cases of pregnant and lying-in women died with age of 17-43 years old, and the average age of pregnant and lying-in women was (30.71±5.82) years old. And 18 cases (58.1%) of pregnant women who died were 25-43 years old. There were 22 cases (71.0%) with a high school or technical secondary school education. The family's per capita annual income was less than 8 000.0 yuan in 22 cases (71.0%). The average health care during pregnancy was (5.12±3.47) times; the top three causes of death were 8 cases of pregnancy complicated with cardiovascular disease (accounting for 25.81%), 5 cases of obstetric hemorrhage (accounting for 16.13%), and 3 cases of pregnancy complicated with liver disease (accounting for 9.68%); Comparing the maternal mortality rates and review results between 2010-2015 and 2016-2020, there was no statistically significant difference in maternal mortality between the two periods (P>0.05); review according to the twelve grid method of the maternal deaths, 20 were avoidable deaths, 10 were unavoidable deaths, and 1 was not reviewed. The top three influencing factors of avoidable death included the link knowledge and skills of medical and health care institutions, the link knowledge and skills of individuals, families and resident groups, and the link management of medical and health care institutions. Conclusions Pregnant women with advanced age, low education level and low family income are the main causes of maternal death in Haikou City. The prevention of high-risk pregnant and lying-in women has moved forward to the pre-marital and pre-pregnancy health care stage. The other is to strengthen the professional and technical level of medical care personnel in medical institutions to effectively reduce the mortality rate of pregnant and lying-in women.
