- Abstract
Leitsymptome zur Gallenwegdiagnostik sind der Oberbauchschmerz, der Ikterus und die laborchemisch manifeste Cholestase. Durch die Entwicklung der kontrastverstärkten Verfahren hat die Sonographie an diagnostischer Treffsicherheit gewonnen, ebenso die Computertomographie durch isotrope Sekundärrekonstruktionen und die Magnetresonanztomographie durch die MR-Cholangiographie nativ und nach leberspezifischem Kontrastmittel sowie diffusionsgewichtete Sequenzen. Diese technischen Verbesserungen erlauben heute auch die Darstellung des nicht dilatierten Gallenwegsystems in diagnostischer Qualität. Normvarianten und Fehlbildungen der biliären Anatomie sind ebenso darstellbar wie der postoperative Situs und damit verbundene Komplikationen. Die Cholelithiasis ist die häufigste Erkrankung der Gallenblase und der Gallenwege. Zusammen mit ihren Komplikationen ist sie damit zugleich in der Mehrzahl der Fälle die Ursache entzündlicher Erkrankungen dieser Organregion. Von besonderer Bedeutung ist dabei wegen der Notwendigkeit zur raschen Therapie der Gallenblasenhydrops.
