Respuesta a tratamiento en pacientes con epilepsia mioclónica juvenil.

- Abstract
Antecedentes: la epilepsia mioclónica juvenil (EMJ) ha sido hasta la actualidad mal diagnosticada y por lo tanto tratada de forma inadecuada. Existen nuevos agentes antiepilépticos que abren las posibilidades de manejo pero que pueden de manera paradójica incrementar las crisis mioclónicas. No existen estudios de evidencia clase I, es decir aleatorizados y controlados para el tratamiento de la EMJ. Objetivos: el presente estudio describe el seguimiento a largo plazo (más de 10 años) de 222 pacientes con EMJ divididos en subsíndromes y analiza la respuesta a tratamientos antiepilépticos incluyendo los factores desencadentantes de las crisis, las crisis que persisten a pesar de tratamiento y los efectos adversos de los antiepilépticos. Material y métodos: se incluyeron 222 pacientes con EMJ que han sido seguidos por el consorcio genes (Genetic Studies Of Epilepsies) a lo largo de un promedio de 10 años. Los pacientes se subdividieron en 4 subsindromes: 1. Epilepsia mioclónica clásica 2. Epilepsia de ausencias de la niñez que evoluciona a EMJ 3. EMJ con ausencias piknolépticas de inicio en la adolescencia 4. EMJ con crisis astáticas durante la adolescencia. Se determinó el tipo de crisis en cada paciente, los factores desencadenantes de las crisis, los antiepilépticos previos y actuales, la presencia o ausencia de crisis en el último seguimiento, el tipo de crisis que persistía a pesar de tratamiento y los efectos adversos de los antiepilépticos. Resultados: 1. EMJ clásica: se incluyeron 161 pacientes, 50% cursaban con crisis mioclónicas y tónico-clónicas generalizadas (TCG), 34% con crisis mioclónicas, TCG y ausencias, 6% con crisis mioclónicas solamente y 1% con mioclonías y ausencias. El 58% se encontraban libres de crisis. Aquellos que tenían más tipos de crisis eran más difíciles de tratar. Los factores desencadenantes de las crisis fueron privación de sueño, el despertar, estrés, uso de alcohol, mal ape- go a tratamiento y la menstruación, el fármaco más ulitizado en los pacientes libres de crisis fue valproato de magnesio en mono o politerapia. El uso de carbamazepina y difenilhidantoína empeoró las crisis en 48% de los pacientes. 2. Epilepsia de ausencias de la niñez que evoluciona a EMJ: se incluyeron 35 pacientes. Los pacientes presentaban ausencias piknolépticas de inicio en la infancia, crisis TCG y mioclónicas. Durante el seguimiento sólo 3 pacientes se encontraban completamente libres de crisis, el resto persistía con crisis convulsivas tanto ausencias, mioclónicas y TCG. 3. EMJ con ausencias piknolépticas de inicio en la adolescencia: se encontraron 18 pacientes con este subsíndrome. Diez se encontraban libres de crisis en tratamiento con valproato de magnesio en mono o politerapia. 4. EMJ con crisis astáticas: ocho pacientes presentaban este subsíndrome. Seis se encontraban libres de crisis recibiendo valproato de magnesio en mono o politerapia. Se encontró adecuada respuesta a fármacos antiepilépticos en todos los subsíndromes excepto el de ausencias de la niñez que evoluciona a mioclónica. Además se encontró que existen fármacos que pueden agravar la EMJ como la carbamazepina y la difenilhidantonía. Se propone en base a los hallazgos de esta serie un algoritmo para el tratamiento de la EMJ.
