Evaluation of Hemocue Albumin 201 System in Comparison to Existing Semiquantitative Dipstick Testing for Urine Albumin Excretion.

- Abstract
Objective: The HemoCue Albumin 201 system is a fast an accurate way of quantitative measuring of urine albumin excretion (UAE) at the point-of-care or the laboratory. The aim of this study was to evaluate the HemoCue Albumin 201 system in comparison to existing semiquantitative dipstick methods for microalbuminuria screening. Subjects-Methods: The study included 165 subjects with various risk factors for cardiovascular and chronic kidney disease, i.e. hypertension, diabetes mellitus and hypercholesterolemia. All subjects gave a first-morning urine sample where albumin was measured with the HemoCue Albumin 201 system, which uses a standard measuring cuvette and a portable photometer and estimates UAE with an immunoturbidometric reaction. The level of UAE was also evaluated with the use of conventional dipsticks (Bayer Clinitek and Roche Micral Test). The sensitivity and specificity of the methods studied were tested against UAE measurement at a central laboratory (Quest Laboratories) with a standard immunoturbidimetric method. A cut-off point of 20 mg/L was used for the detection of elevated UAE to conform to the existing dipstick methods. Results: The prevalence of elevated UAE in the study population was 24%. The HemoCue Albumin 201 system exhibited a sensitivity of 92% and a specificity of 98% for the diagnosis of elevated UAE when compared to the laboratory method. The HemoCue urine albumin measurements were very strongly correlated with the respective laboratory values (r=0.951, P<0.001). The Clinitek dipstick test exhibited a sensitivity of 100% but a specificity of 81% whereas Micral Test dipstick showed sensitivity of 70% and a specificity of 83% for the diagnosis of elevated UAE Conclusions: The HemoCue Albumin 201 system not only provides a valid quantitative estimation of UAE but also has a much better combination of sensitivity and specificity for elevated UAE than existing dipstick tests. HemoCue Albumin 201 could be a very useful system for microalbuminuria detection in the clinical or laboratory settings.
