- Abstract
CASO CLÍNICO Paciente de 26 años que consulta por dolor pélvico que se exacerba con los ciclos menstruales y es remitida a la institución para realización de histeroscopia por hallazgo de útero bicorneo y hematómetras a repetición con antecedente de 4 legrados por lo cual es llevada a Histeroscopia diagnóstica. Hallazgos de histeroscopia Se lleva paciente en agosto 2015 a histeroscopia - laparoscopia diagnostica con hallazgo de cuello central de aproximadamente 3 cm, cavidad uterina única lateralizada hacia la derecha con ostium derecho único permeable, adherencias en el fondo laxas. Pared uterina lateral izquierda completa. Con dichos hallazgos se considera llevar paciente a hemihisterectomía y salpingectomía por vía laparoscópica. Hallazgos de laparoscopia Trompa y ovario derecho de aspecto macroscópico normal, cuerno izquierdo dependiente del fondo uterino no comunicante de aproximadamente 5 cm. Trompa uterina izquierda con adherencias laxas. CONCLUSIONES El útero bicorne representa el 20-40% de las malformaciones uterinas y frecuente asociación a malformaciones urinarias. El diagnóstico de bicorne se puede sospechar por ecografía o histerosalpingografía (HSG), pero el diagnóstico diferencial es exacto con RNM, o la combinación laparoscopia e histeroscopia El abordaje recomendado para el tratamiento de útero bicorne es la laparoscopia.
