In fact, whilst this study did indeed report steam pops in 20% of LSI-guided cases, it also reported steam pops in 23% of the non-LSI-guided arm; hence, the risk was unrelated to the LSI target [[11]]. For example, a lesion with a low CF but prolonged delivery may result in the same LSI as a lesion with high CF but shorter delivery. Ingredients for a perfect radiofrequency lesion Perfect RF delivery should ideally produce a transmural lesion, with contiguity between lesions resulting in effective, bi-directional block. An alternative approach is to utilise a recognised formula for predicting lesion delivery, such as the lesion size index (LSI) or ablation index (AI).
