- Abstract
This article aims to report the experience of monitoring the growth and development of children up to twenty-four months of age in a Family Health Strategy. This is an experience report on actions aimed at child health, carried out in a municipality in the interior of Rio Grande do Sul. The initiative was based on concerns about fragmented care, unsatisfactory monitoring of children and the need to promote changes in the community. Interdisciplinarity, due to its potential to consolidate comprehensive care and more resolute practices, was the guiding principle for developing care for children. Actions include shared childcare consultations, home visits and educational group activities, advocating, above all, health promotion, prevention of diseases, ensure a successful growth and development process, and guarantee the longitudinality of care. The service methodology developed allowed for reflections on how positive it is to break with fragmented professional thinking, integrating knowledge from different areas. The search for and maintenance of interdisciplinary action were challenging and continually mobilized reflections in the team, but also expanded the possibilities and capacities to transform children's health care practices.
