Résumé: Le Covid-19 s'accompagne de troubles de l'hémostase et d'une majoration importante du risque thrombotique. Les tests courants d'hémostase - temps de céphaline avec activateur (TCA), temps de Quick, mesure de la concentration plasmatique de fibrinogène et des D-dimères - sont utilisés pour l'évaluation du risque thrombotique et pour le suivi, mais sont sujets à plusieurs limitations pouvant affecter les résultats. Le suivi du traitement par héparine représente un autre défi pour le laboratoire d'hémostase. Entre autres, les outils pour le suivi de l'administration d'héparine non fractionnée restent l'objet de discussion, et particulièrement dans ce contexte très inflammatoire. Cette revue considère la place des tests d'hémostase dans la prise en charge du Covid-19 et discute leurs principales limitations à prendre en considération. COVID-19 is associated with disturbances of hemostasis in the laboratory and an increased thrombotic risk. Routine laboratory tests - activated partial thromboplastin time (aPTT), prothrombin time, Clauss fibrinogen and D-dimers levels measurement - are used for the evaluation of the thrombotic risk and the monitoring of hemostasis, but are subject to several drawbacks that may affect the reliability and clinical relevance of the delivered results. Another challenge for the hemostasis laboratory is the monitoring of heparin treatment. For instance, the issue of the monitoring of unfractionated heparin remains debated, the more so when there is a tremendous inflammatory response. This brief review considers the role of laboratory tests of hemostasis in the management of COVID-19 and discusses their main limitations to be kept in mind.
