- Abstract
Introduction: Angiomyolipoma (AML) is a benign tumor which occurs almost exclusively in the kidney. Patients with AML, that exhibit spontaneous bleeding or other symptoms, such as pain, should receive treatment. Transarterial embolization (TAE) is one of the main treatment options for symptomatic AML. However the role of TAE as a means of preventing AML hemorrhage, is a subject of debate. Purpose: The purpose of this study is to compare the timing, effectiveness, safety and cost between urgent and prophylactic embolization of renal AML. Material and methods: We retrospectively examined seven consecutive cases of TAE of renal AML, which were performed between March 2009 and March 2012. Group 1 (urgent TAE) included 5 patients (age, mean; 40,2 years, mean AML diameter; 11,06 cm) with retroperitoneal hematoma from ruptured AML, who required immediate or delayed transfusion (an average of 2,66 units per patient). Group 2 (prophylactic TAE) included 5 asymptomatic patients (age, mean; 48,1 years, mean AML diameter; 5,62 cm). All patients underwent superselective TAE and were monitored clinically for the first two weeks and one month after embolization. Results: Embolization was technically successful in all cases. There was one case of group 1, which required repeat embolization. There were no serious immediate complications. The post-embolization fever did not exceed 38,5 °C and lasted on average 10.3 days and 12 days in groups 1 and 2 respectively. One case of group 1, also needed a blood transfusion during hospitalization. After a month, the average reduction in the volume of AML was 14% for group 1 and 19% for group 2. Conclusions: Prophylactic embolization of renal AML appears to be superior to urgent embolization in terms of duration of hospitalization, efficiency and overall cost of treatment.
