540 Continuous Toxicity Monitoring in Phase II Trials in Oncology Le but des essais de phase II en oncologie est d'évaluer l'efficacité d'un nouveau traitement et la dose étudiée est généralement la dose maximale tolérée telle qu'elle a été estimée préalablement dans un essai de phase I. En raison du manque de précision de cette estimation, de nombreux essais de phase II utilisent des règles d'arrêt pour toxicité. Nous montrons comment construire des règles d'arrêt séquentielles pour la surveillance en continu de la toxicité. Nous fournissons une table qui permet d'obtenir facilement les frontières, du type de celles de Pocock, pour diverses valeurs du pourcentage de toxicité et diverses tailles d'échantillon. Nous discutons l'estimation des pourcentages de toxicité et de réponse au traitement. 546 A Note on -Values under Group Sequential Testing and Nonproportional Hazards Des schémas séquentiels groupés sont souvent utilisés pour évaluer périodiquement l'efficacité d'un traitement au cours d'un essai clinique. A la suite d'un test séquentiel groupé, les P-valeurs calculées sous l'hypothèse que les données ont été collectées selon un schéma fixe ne suivent plus une distribution uniforme sous l'hypothèse nulle d'absence d'effet traitement. Diverses relations d'ordre ont été proposées pour calculer les P-valeurs correctes après un test séquentiel groupé. Bien que beaucoup des relations d'ordre proposées aient été comparées dans le cadre d'effets des traitement invariants dans le temps, peu d'attention a été donnée à leur performance lorsque l'effet du traitement chez un individu varie avec le temps. Notre intérêt ici est de comparer deux des méthodes les plus couramment utilisées pour calculer les P-valeurs correctes, reposant sur le temps de l'analyse et la statistique Z, au regard des fonctions de puissance lorsque les effets des traitements sur la survie sont retardés. La puissance sous la relation d'ordre reposant sur le temps d'analyse est montrée être fortement influencée par l'existence d'un effet traitement retardé, alors que la puissance sous la relation d'ordre reposant sur la statistique Z reste stable avec des effets retardés des traitements. 552 Bayesian Cure Rate Frailty Models with Application to a Root Canal Therapy Study Liéà des clusters naturels ou artificiels, des données de survie multivariées sont souvent rencontrées dans les études biomédicales, par exemple, une étude dentaire impliquant plusieurs dents pour chaque sujet. Un certain pourcentage de sujets dans la population pour lesquels ont ne s'attend pas à ce qu'ils présentent l'événement d'intérêt sont considérés comme « guéris » ou non susceptibles. Ces entrées tardives sont traitées, dans la littérature statistique, comme des données tronquées à gauche. Cependant, avec des données réelles, il existe une possibilité substantielle que ces entrées tardives aient des risques extrêmement différents de celui des sujets standards. Nous nous focaliserons sur une situation dans laquelle de tels biais d'entrée peuvent arriver dans l'analyse de données de survie. L'objectif de cet article est de développer une méthodologie appropriée pour faire de l'inférence sur des données incluant des entrées tardives. Nous construisons un modèle qui inclut des paramètres pour l'effet du biais dûà une entrée tardive sans aucune spécification sur la distribution du temps d'entrée. Nous discutons aussi de l'inférence de la vraisemblance basée sur ce modèle et dérivons le comportement asymptotique des estimateurs. Une étude de simulation est conduite pour un échantillon de taille finie afin de comparer les résultats de l'analyse utilisant notre méthode à ceux obtenus en utilisant les méthodes usuelles, où l'indépendance entre le temps d'entrée et le temps de défaillance est supposée. Nous appliquons notre méthode à une analyse de la mortalité parmi les survivants d'une attaque atomique dans une région géographique d'étude bien définie. 567 Semiparametric Analysis of Survival Data with Left Truncation and Dependent Right Censoring L'étude des maladies chroniques implique souvent simultanément celle de la mortalité et de la morbidité. Dans les études d'observations, les données peuvent être sujettes à de la troncature à gauche et de la censure à droite. Du fait que mortalité et morbidité peuvent être corrélées et que la mortalité peut être cause de censure pour la morbidité, l'estimateur de Lynden-Bell pour les données tronquées à gauche et censurées à droite peut être biaisé si l'on estime la fonction de survie marginale de l'événement non terminal. Nous proposons un estimateur semi paramétrique pour cette fonction de survie, fondé sur un modèle contenant ces deux durées, qui utilise une spécification gamma de la fragilité dans le domaine des données observables. Tout d'abord, nous développons un nouvel estimateur du paramètre de fragilité en présence de troncature à gauche. Utilisant cet estimateur, nous obtenons ensuite un estimateur compact pour la distribution marginale de l'événement non terminal. Les propriétés de cet estimateur pour de grands échantillons sont établies à partir de la théorie asymptotique. La méthode donne de bons résultats sur des échantillons de taille modérée que ce soit lors de simulations ou dans l'analyse de données issues d'un registre du diabète. 576 A Varying-Coefficient Cox Model for the Effect of Age at a Marker Event on Age at Menopause En ce qui concerne la recherche médicale dans le domaine de l'aide à la procréation, il est intéressant d'examiner la validité d'un marqueur événement utilisé pour estimer le début de la transition à l'état de ménopause et pour estimer l'age à la ménopause en utilisant l'age au marqueur événement. Nous proposons un modèle de Cox avec des covariables dépendants du temps pour étudier l'association entre l'age au marqueur événement, défini comme une évolution caractéristique de saignements spécifiques, et l'age à la ménopause, où, à la fois, les deux événements sont sujets à la censure et leur association varie avec l'age au marqueur événement. La procédure d'estimation utilise une régression spline. La méthode proposée est appliquée à des données « Tremin Trust » pour évaluer l'association entre l'age du début du cycle menstruel de 60 jours et l'age à la ménopause. La performance de la méthode proposée est évaluée grâce à une étude de simulation. 584 On a Supplemented Case–Control Design Le schéma cas-témoin augmenté consiste en un échantillon cas-témoin et un échantillon supplémentaire de sujets non malades, issu d'une strate relative à une des expositions mesurées dans l'étude cas-témoin. Les données supplémentaires peuvent provenir, par exemple, d'une enquête conduite indépendamment de l'étude cas-témoin. Ce schéma améliore la précision des estimateurs des effets principaux et spécialement des expositions simultanées, en particulier lorsque ces dernières sont rares et que la prévalence d'une des expositions est basse. Nous adaptons ensuite les méthodes de schémas à deux phases pour établir des estimateurs de la vraisemblance pour les logarithmes des rapports des chances dans ce schéma et dériver des estimateurs asymptotiques des variances qui rendent compte de façon appropriée des différences entre les modes d'échantillonnage de ce schéma et ceux du classique schéma à deux phases. A titre d'illustration de notre schéma, nous présentons une étude conduite pour évaluer l'influence d'une exposition simultanée à l'infection par le virus de l'hépatite B et par celui de l'hépatite C sur le risque de cancer hépatocellulaire à partir de données de la région de Qidong, province de Jiangsu, Chine. 591 Exploratory Bayesian Model Selection for Serial Genetics Data La caractérisation du processus par lequel des changements au niveau moléculaire et cellulaire surviennent au cours du temps aura de larges implications dans la prise de décision clinique et aidera notre connaissance de l'étiologie de la maladie à travers de nombreuses maladies complexes. Cependant cela présente un défi analytique dûà la fois au nombre important de biomarqueurs potentiellement d'intérêt et aux relations complexes et non caractérisées existantes entre eux. Nous proposons une procédure bayesienne exploratoire de sélection de modèles, qui cherche la simplicité du modèle via des tests d'indépendance entre les multiples biomarqueurs discrets mesurés au cours du temps. Les calculs de facteurs de Bayes sont utilisés pour identifier et comparer les modèles qui sont les mieux ajustés aux données. Dans de grands espaces de modèles, i.e., un grand nombre de biomarqueurs multi-niveaux, nous proposons un algorithme de recherche stochastique de Monte-Carlo par Chaîne de Markov pour trouver les modèles promis. Nous appliquons notre procédure à l'étude approfondie de la façon dont laquelle les changements génétiques de l'HIV-1 apparaissent indépendamment du temps. 600 Nonparametric Estimation of ROC Curves in the Absence of a Gold Standard Pour l'évaluation du degré de fidélité diagnostique de tests, un étalon de référence sur l'état de la maladie est nécessaire. Cependant dans de nombreuses maladies complexes, il est impossible voire non éthique d'obtenir un tel étalon de standardisation. Si un standard imparfait était utilisé, le degré de fiabilité estimé des tests pourrait être biaisé. Ce type de biais est appelé biais de référence imparfaite. Dans cet article, nous développons une méthode de maximum de vraisemblance non-paramétrique pour l'estimation de courbes ROC et de leurs surfaces pour des tests àéchelle ordinale en l'absence de référence. Notre étude de simulation montre que les estimateurs proposés pour les surfaces sous courbe ROC ont de bonnes propriétés à distance finie, en terme de biais et d'erreur quadratique moyenne. D'autres études de simulation montrent que notre approche non-paramétrique est comparable à la méthode paramétrique binormale, et plus aisée à implémenter. Enfin nous appliquons la méthode proposée à une étude clinique réelle sur l'exactitude de sept anatomo-pathologistes pour la détection du cancer du col utérin in-situ. 610 Quantifying Uncertainty Associated with Microbial Count Data: A Bayesian Approach Nous considérons le problème de l'estimation d'une concentration bactérienne, à partir d'un dénombrement de microorgansimes. Nous proposons une approche bayésienne qui permet de prendre en compte à la fois les dénombrements et les résultats des tests de confirmation comme un génotypage par PCR. Contrairement aux méthodes antérieures qui ne fournissaient généralement que des estimations ponctuelles, nous obtenons des intervalles de crédibilité a posteriori pour les concentrations bactériennes. Nous illustrons la méthode avec des données concernant le dénombrement par étalement en spirale d'un pathogène alimentaire, Escherichia coli O157, mais notre approche s'applique à n'importe quel microorganisme et à n'importe quelle méthode de mesure. Nos résultats permettent d'éclairer les expérimentateurs quant au nombre de tests de confirmation à effectuer; ils suggèrent également que lors du dénombrement initial, il vaut mieux inclure qu'exclure les colonies de génotype douteux. 617 On Geodetic Distance Computations in Spatial Modeling Les statisticiens analysant des données spatiales cherchent souvent à prédire et modéliser des associations basées sur des distances mesurées à la surface du globe terrestre. Un calcul précis des distances est souhaitable, d'abord pour faciliter l'exploration et l'interprétation des données, ensuite pour développer des algorithmes d'estimation stables. Lorsque les données sont exprimées en positions à la surface du globe, le passage à des distances euclidiennes ou à une métrique plane n'est pas trivial. Les métriques planes sont cependant désirables à cause de leur facilité d'interprétation, de leur grande disponibilité dans la plupart des logiciels courants, et de leurs propriétés théoriques connues depuis longtemps. Bien que les calculs de distances soient indispensables pour la modélisation géostatistique, leur importance et leurs impacts sur les estimations et prédictions statistiques n'ont pratiquement jamais étéétudiés. Cet article explore les différentes options de l'utilisation des métriques planes et examine leur impact sur la modélisation géostatistique.
